左乙拉西坦片对老年癫痫患者相关血清指标及疗效的影响

摘　　要：目的探讨左乙拉西坦片辅助治疗老年癫痫的临床疗效及对血清TNF-α、IL-2、IL-6、ICAM-1的影响。方法选择2016年4月~2017年7月在我院接受治疗的老年癫痫患者90例,按照随机数表法分为观察组(45例)和对照组(45例),对照组给予卡马西平片联合苯巴必妥片治疗,观察组在对照组基础加用左乙拉西坦片治疗,两组均治疗6个月。治疗后观察其临床疗效,比较两组患者治疗前后血清TNF-α、IL-2、IL-6、ICAM-1水平,以及GCS、Barthel指数及MoCA评分。结果治疗后,观察组总有效率明显高于对照组(P<0.05);观察组血清TNF-α、IL-2、IL-6、ICAM-1水平均显著低于对照组,GCS、Barthel指数及MoCA评分均显著高于对照组,两组比较差异有统计学意义(P<0.05)。结论左乙拉西坦辅助治疗老年癫痫可有效改善患者昏迷程度、日常生活能力以及认知功能,降低患者血清TNF-α、IL-2、IL-6、ICAM-1水平,具有显著的临床疗效,安全性好,可在临床推广应用。Objective To study the curative efficacy of levetiracetam add-on therapy in treatment of elderly epileptic patients and its effects on TNF-α, IL-2, IL-6 and ICAM-1. Methods 80 patients of elderly epileptic received therapy from April 2016 and July 2016 in our hospital were selected as research objects, according to random table method with, those patients were divided into observation group （45 cases） and control group （45 cases）. The control group was treated with carbamazepine combined with Phenobarbital. On the basis of the control group, the observation group was treated with levetiracetam, both groups were treated for 6 months. After treatment, the clinical efficacy, the serum TNF-α, IL -2, IL-6, ICAM-1, and GCS, Barthel index, and MoCA scores were observed. Results After treatment, the total effective rate of observation group （87.50%） was higher than that in the control group （66.67%） （P〈0.05）. The levels of TNF-α, IL-2, IL-6 and ICAM-1 in the observation group were less than the control group, and the GCS, Barthel index and MoCA score were higher than the control group, and the difference between the two groups was significant （P〈 0.05）. Conclusion Levetiracetam add-on therapy in treatment of elderly epileptic patients can effectively improve the degree of coma, daily life ability and cognitive function, reduce the patients serum TNF-α, IL-2, IL-6, ICAM-1 level and has significant clinical efficacy and safety.
