青年出血性脑卒中病因探讨

摘　　要：目的探讨青年出血性脑卒中的病因,为临床诊疗提供预防措施。方法选取西安交通大学第一附属医院神经内科住院的青年出血性脑卒中病历240份,采用回顾性分析方法,汇总其病因。结果病因明确184例(76.67%),包括高血压病58例(24.17%),脑动静脉畸形44例(18.33%),颅内动脉瘤34例(14.17%),血液系统疾病共15例(6.25%),瘤卒中共4例(1.67%),脑海绵状血管瘤8例(3.33%),妊娠产褥期2例(0.83%),烟雾病14例(5.84%),静脉窦血栓形成2例(0.83%),硬脑膜动静脉瘘2例(0.83%),结缔组织病1例(0.42%),原因不明56例(23.33%)。结论高血压是青年出血性脑卒中的最主要病因,动脉瘤及动静脉畸形是第二病因。Objective To provide prevention methods for clinical diagnosis and treatment by detecting the etiology of youth hemorrhagic stroke. Methods 240 patients with youth cerebral hemorrhage from Department of Neurology, The First Affiliated Hospital of XI＇ an Jiaotong University were taken and analyzed. Results There were 184 cases （76.67%） with definite etiology, including essential hypertension （24.17%）, cerebral arteriovenous malformation （18.33%）, intracranialaneurysm （14.17%）, blood system diseases （6.25%）, tumor stroke（1.67%）, cavernous hemangioma （3.33%）, puerperal pregnancy（0.83%）, moyamoya disease （5.84%）, venous sinus thrombosis （0. 83%）, dural arteriovenous fistula （0.83%） and connective tissue disease （0.42%）. There were 56 cases with unknown reasons （23.33%）. Conclusion Hypertension is the main etiology of youth cerebral hemorrhage. The aneurysm and arteriovenous malformation were the second cause.
