摘  要:[目的]研究微创治疗外翻周围软组织对截骨端稳定性的影响。[方法]建立外翻足三维有限元模型并进行可靠性验证,模型包括骨骼、软骨、肌腱、足内在肌和韧带等组织材料。利用CATIA软件模型上模拟第1跖骨头颈部截骨,并向外侧推移。模拟外翻患者在平衡站立时,截骨端周围有无软组织(肌肉、肌腱和韧带)存在两种情况下的工况,分别计算截骨端的应力和总位移,对结果进行对比分析。[结果]无软组织工况截骨断端间最大Von Mises应力为0.296 Mpa,应力主要集中在截骨背侧远端,并向近端逐渐减小;有软组织工况截骨断端间最大Von Mises应力为0.468 Mpa,应力主要集中在整个截骨面上,并向近端和跖侧集中。无软组织工况截骨端总位移为0.336 mm,有软组织工况为0.264 7 mm,两种工况的位移都发生在整个截骨面上。[结论]微创治疗外翻截骨端软组织能减少截骨端移位,增加截骨端压应力,有利于截骨端的稳定和愈合。[Objective] To investigate the effect of soft tissue on stability of minimally invasive osteotomy for hallux valgus correction. [Methods] Three dimensional finite element model of hallux valgus was established to verify the reliability. The model included bone, cartilage, tendon, foot, inner muscle, ligament and other tissue. The osteotomy and lateral movement of the head of first metatarsal bone was simulated using the CATIA software. As the patient with hallux valgus in balanced standing, two conditions with or without soft tissue (muscle, tendon and ligament) around the osteotomy were simulated, then calculated stress and displacement of the osteotomy.
