摘  要:目的探讨急性腹膜炎对大鼠罗库溴铵腹肌肌肉松弛效应和腹直肌功能的影响。方法雄性Sprague-Dawley大鼠36只,体重220~250g,采用随机数字表法将大鼠分为假手术组、胃穿孔1h组、胃穿孔2h组,每组12只。参照文献制作胃穿孔急腹症模型,腹腔内注入气体并记录压力变化,绘制容量-压力曲线,完成后固定腹腔压力在15mmHg(1mmHg=0.133kPa),压力无变化后经尾静脉注入2倍90%有效剂量的罗库溴铵(3.5mg/kg),记录1、5和10min时腹腔压力变化情况,记录呼吸恢复时间。压力测定完成,大鼠呼吸恢复后心脏采血,采用ELISA法测定血浆IL-6、TNF-α、IL-13含量。取左腹直肌条,测定单刺激肌颤搐、半舒张时间(1/2RT)、收缩时间(TPT)、最大强直收缩力和疲劳指数(FI),以及给予肌条10、20、40、60、80、100和120Hz定时刺激时的膈肌张力,绘制频率-张力曲线。所有力学参数采用肌条横截面积(CSA)进行校正。结果胃穿孔2h组术后的IL-6和TNF-α水平均显著高于假手术组(P值均<0.01);IL-13水平轻度升高,但与假手术组的差异无统计学意义(P>0.05)。胃穿孔1h组和胃穿孔2h组的容量-压力曲线较假手术组明显上移。胃穿孔1h组和胃穿孔2h组在60、70、80、90、95和100mL气体容积时的腹腔压力均显著高于假手术组同气体容积时(P值均<0.05);给予罗库溴铵后1、5和10min时,假手术组大鼠的腹腔压力均显著低于胃穿孔1h组和胃穿孔2h组(P值均<0.01);3组间呼吸恢复时间的差异均无统计学意义(P值均>0.05)。3组大鼠间体重、肌条质量、肌条最初长度和CSA的差异均无统计学意义(P值均>0.05)。胃穿孔2h组的腹直肌单刺激肌颤搐和最大强直收缩力均显著高于假手术组(P值均<0.05),1/2RT显著长于假手术组(P<0.01);3组间TPT的差异无统计学意义(P值均>0.05);胃穿孔1h组和胃穿孔2h组的FI均显著低于假手术组(P值分别<0.05、0.01)。给予腹直肌肌条10、20、40、60、80、10Objective To evaluate the effect of rocuronium on abdominis muscles and the function of rectus abdominis in acute peritonitis in rats. Methods A total of 36 adult male Sprague-Dawley rats, weighing 220- 250 g, were divided randomly into three groups (n = 12). sham operation group (group S), one-hour gastric perforation (group G1), and two-hour gastric perforation (group G2). Acute peritonitis was induced by artificial gastric perforation (GP) from literatures in group G1 and G2. The air was injected into the abdominal cavity and the curve of pressure-volume was drawn. When the pressure of the abdominal cavity maintained at a constant value of 15 mmHg (1 mmHg = 0. 133 kPa), rocuronium (3.5 mg/kg) was infused by the tail vein. The Change of intra- abdominal pressure (IAP) at 1, 5 and 10 min after rocuronium injection and the recovery duration of spontaneousbreath were recorded. Then the blood samples were collected from the animal's heart, and the levels of inter leukin (IL)-6, tumor necrosis factor (TNF)-α and iL-13 were measured. The left rectus abdominis was used to detect the systolic and diastolic function, including the single stimulation twitch, maximal force of tetanic contraction, half relaxation time (1/2RT), contraction time (TPT) and fatigue index (FI). The force-frequency curve was drawn according to the data from the measurement of muscle strip tensions stimulated at 7 different frequencies from 10- 120 Hz in 10 Hz increments. All force values were normalized to cross-sectional areas (CSA). Results The serum levels of IL-6 and TNF-α in the G2 group were significantly higher than those in the group S (both P〈0. 01 ). The IL-13 level was slightly increased in the G2 group, which was not significantly different from that in the group S (P〉0.05).
