卒中后抑郁与脑损伤部位相关性的临床研究

摘　　要：目的:研究脑损伤部位与卒中后抑郁(PSD)之间的相关性。方法:收集本院100例脑卒中患者的临床资料,分析脑卒中并发抑郁的发病率、抑郁组与非抑郁组脑卒中患者脑损伤部位相关性。结果:PSD率为39.00%(轻度抑郁66.67%,中度抑郁28.21%,重度抑郁5.12%)。神经功能缺损(MSSS)分数越高脑PSD越重。非抑郁组与抑郁组患者脑出血容积、脑梗死损伤最大直径之间,无统计学意义,即脑出血或脑梗死损伤范围极有可能与抑郁发病不存在关系;右侧大脑半球与左侧大脑半球,大脑半球后部与前部,皮层下与皮层之间发生的脑卒中例数比有统计学意义(P<0.05),即左侧半球损伤、大脑半球皮层损伤,大脑半球前部损伤的脑卒中患者并发抑郁率高。结论:脑PSD属于常见的一种并发症,即脑MSSS评分越高发生抑郁程度越重。Objective： To study the correlation between brain injury site and post-stroke depression（PSD）. Methods： The clinical data of 100 stroke patients in our hospital were collected to analyze the incidence of stroke complicated by depression, and the correlation between brain injury in stroke patients and non-depression patients. Results： The PSD rate was 39.00%（mild depression 66.67%, moderate depression 28.21%, severe depression 5.12%）. The higher the neurological deficit（MSSS） score, the greater the PSD of the brain. No significant difference was found between the volume of cerebral hemorrhage and the maximum diameter of cerebral infarction in non-depressive group and depression group, that is, the range of cerebral hemorrhage or cerebral infarction was most likely not related to the onset of depression. The relationship between right hemis phere and left brain Cerebral hemispheres, posterior hemispheres and anterior, subcortical and cortical stroke cases were statistically significant（P〈 0.05）, that is, left hemisphere injury, cerebral cortex damage, anterior hemispheric injury Stroke patients with high depression rate. Conclusion： Brain PSD is a common complication, that is, the higher the MSSS score is, the more depressive one is.
