摘 要:目的调查研究早产儿血清维生素A水平,为临床指导孕妇和早产儿合理补充维生素A提供参考依据。方法选取2016年1月-12月在武汉科技大学附属孝感市中心医院产科出生的胎龄28~36+6周早产儿为研究对象,分析不同性别、胎次、分娩方式、胎龄、体重、季节的血清维生素A水平。结果武汉科技大学附属孝感市中心医院早产儿的血清维生素A总体缺乏率为62.47(303/485),不同性别、胎次、分娩方式的早产儿血清维生素A水平比较差异无统计学意义(均P>0.05);早期早产儿、中期早产儿、晚期早产儿血清维生素A水平依次增高,差异有统计学意义(均P<0.05);超低出生体重儿、极低出生体重儿、低出生体重儿、正常出生体重儿血清维生素A水平依次增高,差异有统计学意义(均P<0.05)。早产儿血清维生素A水平与出生胎龄、体重呈正相关(r=0.094、0.296,均P<0.05),但与出生季节无相关性(r=0.000,P>0.05)。结论早产儿维生素A缺乏率偏高,应注重早产儿早期维生素A的监测和补充。Objective To survey the levels of serum vitamin A in premature infants,provide a reference basis for clinical guidance of reasonable supplementation of vitamin A in pregnant women and premature infants. Methods The premature infants with fetal age 28-36+6 weeks born in Department of Obstetrics in Xiaogan Central Hospital Affiliated to Wuhan University of Science and Technology from January to December in 2016 were selected as research object,the levels of serum vitamin A in premature infants of different genders,delivery times,delivery modes,weights,and seasons were analyzed. Results The total incidence rate of serum vitamin A deficiency in premature infants was 62. 47%( 303/485). There was no statistically significant difference in serum vitamin A level among the premature infants of different genders,delivery times,and delivery modes( P〉0. 05). The levels of serum vitamin A in early premature infants,moderate premature infants,and late premature infants increased gradually,there were statistically significant differences( P〉0. 05). The levels of serum vitamin A in extremely low birth weight infants,very low birth weight infants,low birth weight infants,and normal weight infants increased gradually,there were statistically significant differences( P〈0. 05). Serum vitamin A level in premature infants was positively correlated with gestational age and birth weight( r = 0. 094,P〈0. 05; r = 0. 296,P〈0. 05),but it was not correlated with season( r = 0. 000,P〉0. 05). Conclusion The incidence rate of vitamin A deficiency in premature infants is relatively high,more attention should be paid to early monitoring and supplemantation of vitamin A in premature infants.
