龟头鳞状细胞癌伴双侧腹股沟鱼嘴样溃疡转移1例

摘　　要：患者男,67岁。双侧腹股沟结节3个月,破溃2个月。体检:双侧腹股沟对称性窦道,开口长轴3cm,深度约5cm,上覆少量黄色稀薄分泌物,伴恶臭。盆腔MRI平扫示双侧腹股沟区及龟头异常信号。进一步检查示包皮龟头粘连,龟头侧面见一黄豆大新生物。窦道边缘及龟头肿物活检均显示为高分化鳞癌,免疫组织化学均为CK5/6(+),P40(+)。最终诊断:阴茎龟头鳞状细胞癌并双侧腹股沟转移。A 67-year-old male presented with the bilateral inguinal nodules for three months and ulcer for two months. Dermatological examination showed symmetrically bilateral in- guinal sinuses, with a longitudinal axis of 3cm and 5cm infiltrated depth, covered with foul odor, thin yellow secretion. In the subsequent investigation, MRI showed abnormal signals of bilateral inguinal region and glans penis, and the further examina- tion showed adhesion of prepuce and glans, with a soybean-size neoplasm on the glans penis. Histopathologic examinations of biopsies from both left groin ulcer and glans penis revealed well-differentiated squamous cell carcinoma, and the immunohisto- chemistry examinations showed CK5 （ ＋ ）, CK6 （ ＋ ）and P40 （ ＋ ）. The final diagno- sis was squamous cell carcinoma of the balanus complicated by the metastasis of bilat- eral inguina.
