摘 要:目的观察缺氧诱导因子(hypoxia inducible factor-1,HIF-1α)、转化生长因子(transforming growth factor-β,TGF-β)在尿路移行上皮癌中的表达,确认2个细胞因子与尿路移行上皮癌进展的相关性。方法收集低级别移行上皮癌30例、高级别移行上皮癌60例(浸润内1/2和外1/2肌层各30例),采用免疫组化和组织芯片技术观察肿瘤组织中HIF-1α、TGF-β的表达,并与其临床病理行相关性分析。结果 (1)HIF-1α在低级别和高级别癌中均表达,阳性率分别为33%(10/30)和67%(40/60),差别具有统计学意义(P<0.05);TGF-β在低级别和高级别癌中均表达,阳性率分别为30%(9/30)和70%(42/60),差别具有统计学意义(P<0.05);(2)HIF-1α表达水平与组织学类型、浸润深度、淋巴结转移相关(P<0.05),与肿瘤体积、脉管内癌栓、TNM分期无关(P>0.05);TGF-β表达水平与肿瘤体积、脉管内癌栓、组织学类型密切相关(P<0.05),与淋巴结转移、TNM分期、浸润深度无关(P>0.05)。(3)HIF-1α、TGF-β表达水平在低级别尿路移行上皮癌中无相关性(r=0.2802,P>0.05);在高级别尿路移行上皮癌中呈正相关(r=0.7803,P<0.05)。结论 HIF-1α和TGF-β在移行上皮癌中均存表达,随移行上皮癌恶性级别升高而表达明显增强,提示HIF-1α和TGF-β与移行上皮癌的进展密切相关。Objective To study the role of the expression of the hypoxia inducing factor(HIF-1α ),transforming growth factor(TGF-β)in transitional epithelial tumor progression.Methods To collect the low grade of urothelial carcinoma(30 cases),high grade of urothelial carcinoma(infiltration within 1/2 and 1/2 muscularis each 30 cases respectively),all with complete clinical and pathological data,using the immunohistochemical and contrast groups chip technology to detect the expression of HIF-1α and TGF-βin tumor tissue,and at the same time comparative research its clinical pathological parameters.Results(1)There are the expression of HIF-1α in low grade and high grade tumor,the positive rate was 33%(10/30)and 67%(40/60)respectively,there are statistically significant difference between both(P〈0.05);The positive rate of TGF-βin low grade and high grade tumor was 30%(9/30)and 70%(42/60)respectively,there are statistically significant difference between both(P〈0.05);(2)There are closely correlationship among the expression level of HIF-1α and histological type,infiltration depth,lymph node metastasis(P〈0.05),and there are no correlationship among the expression level of HIF-1α and tumor volume,vascular tumor emboli,TNM staging(P〈0.05);there are correlationship among the expression level of TGF-βand the tumor volume,vascular tumor emboli,histologic type(P〈0.05),there are no correlationship among the expression level of the TGF-βand lymph node metastasis,TNM staging,infiltration depth(P〉0.05).(3)there are no correlationship between the expression level of HIF-1α and TGF-βat the low grade urothelial carcinoma(r=0.2802,P〉0.2802);there are correlationship at the high grade urothelial carcinoma(r=0.7803,P〈0.05).Conclusion there are both the expression of HIF-1α and TGF-βat urothelial carcinoma and then increasing expression with the raise the grade of urothelial carcinoma grade,there are the close correlationship between the expre
