摘 要:目的研究探讨六价铬化物热液烫伤后,SD大鼠肝肾功能的急性损害程度、具体变化,并与普通热液烫伤进行对比,了解其特殊性,为烧伤外科临床治疗六价铬化物热液烫伤提供一定的参考依据。方法将30只SD大鼠按照完全随机数字表法分为3组,每组10只,分别建立30%TBSAⅢ度烫伤大鼠模型,分别是六价铬化物热液烫伤组(以下称铬化物热液烫伤组)、普通热液烫伤组及假烫对照组。动物模型制备成功后,分别于伤后2h、8h、24h、48h采集血液样本,收集血清,及时测定肝肾功能主要指标。判断其在不同时相的变化规律,对比差异,进行相关性分析。结果烫伤后,不同时相点各项肝肾功能主要指标均异常于假烫组;普通热液烫伤组、铬化物热液烫伤组SD大鼠血清白蛋白均低于假烫组,但2组之间在各时相点并无明显差异(P>0.05),烫伤后8h开始,铬化物热液烫伤组SD大鼠谷草转氨酶及谷丙转氨酶明显高于普通热液烫伤组(P<0.01);铬化物热液烫伤组伤后2h开始,血尿素氮即明显高于普通热液烫伤组(P<0.05),伤后24h、48h血肌酐明显高于普通热液烫伤组(P<0.01)。伤后24h、48h铬化物热液烫伤组血尿酸明显高于普通热液烫伤组(P<0.01)。结论六价铬化物热液烫伤与普通热液烫伤的SD大鼠血清所反映的肝肾功能的主要指标总体提示其肝肾功能损伤明显异常于假烫伤SD大鼠,但六价铬化物热液烫伤要比普通热液烫伤造成的急性肝肾功能损害严重,这为在烧伤外科临床治疗六价铬化物热液烫伤提供了参考和需要关注的治疗重点。Objective To investigate the changes of hepatic and renal function in hexavalent chromium compounds hot liquid burned SD rat during earlier period,study the regularities and the significances,in order to provide reference in burn clinic.Methods Thirty SD rats were divided into three groups by random digits table.There were 10 rats in each group.The three groups were scalded by hexavalent chromium compounds hot liquid,ordinary hot liquid and ordinary warm water.We collected the blood samples of SD rats after animal model finished.After the indexes of hepatic and renal function were tested,we analyzed the results and tried to find the regularities.Results The hepatic and renal function indexes were different from those of sham scald group at different time points.The serum albumin levels in SD rats were significantly lower than those in sham scald group,but there was no significant difference between the 2 groups at different time points.8 hafter scald,chromium compounds hot liquid scald group SD rat aspartate aminotransferase and alanine aminotransferase was significantly higher than that of normal scald group.In the scalded group,the blood urea nitrogen was significantly higher than that of the conventional scald group after 2 h.The serum creatinine of24 hand 48 hafter injury was significantly higher than that of the ordinary scald group.The serum uric acid levels of 24 h and 48 hchromium in the scald group were significantly higher than those in the ordinary scald group.ConclusionThe main indicators of serum SD rats with high temperature sodium dichromate hot liquid scald and hot liquid scald reflected by the hepatic and renal function suggesting that the overall damage to hepatic was abnormal in sham scalded SD rats,hepatic function of acute renal injury but high temperature sodium dichromate hot liquid scald than hot liquid scald cause serious for this,in the clinical treatment of burn surgery chromium compounds hot liquid scald provides the reference and the need to focus on the key treatment.
