激光断线在青光眼小梁切除术后的应用

摘　　要：目的研究激光断线在青光眼小梁切除术后的应用。方法回顾性分析2013年12月至2015年12月原发性慢性闭角型青光眼手术58例（61眼）的临床资料。患者随机分为两组，A组28眼，行小梁切除联合巩膜瓣固定缝线缝合术，B组33眼，行小梁切除联合术后激光断线。观察术前术后眼压和术后并发症。术后随访3个月。结果两组术前眼压比较差异无统计学意义（P〉0．05），术后1周B组眼压高于A组，差异有统计学意义（P〈0．05），但是术后1个月、3个月两组比较差异均无统计学意义（P〉0．05）。术后3个月随访时A组总成功率为75．0％，B组总成功率为84．8％，差异无统计学意义（P〉0．05）。术后早期浅前房、低眼压等并发症A组患者发生较多，两组差异有统计学意义（P〈0．05）。结论小梁切除术后应用激光断线能够减少浅前房、低眼压等术后早期并发症。Objective To study the application of laser suture lysis after trabeculectomy. Methods The data of 61 eyes of 58 patients with primary chronic angle-closure glaucoma from Dec. 2013 to Dec. 2015 were retrospectively analyzed. The patients were randomly divided into two groups. Group A, 28 eyes were treated with trabeculectomy and scleral flap suture fixation. Group B, 33 eyes were treated with trabeculectomy and laser suture lysis. Preoperative and postoperative intraocular pressure and postoperative complications were observed. The follow-up time was 3 months. Results The difference was not significant in the preoperative IOP between the two groups （P 〉 0.05 ）. One week after operation, the lOP in group B was higher than that in group A （ P 〈 0.05 ）, but the difference was not statistically significant between the two groups （ P 〉 0.05 ） in 1 and 3 months after operation. At 3 months follow-up, the total success rate in group A was 75%, and 84.8% in group B, the difference was not significant in the total success rate between group A and group B （P 〉0. 05）. Shallow anterior chamber, bypotony and other early complications occurred often in group A early postoperatively, and the difference was significant between the two groups （P 〈 0.05）. Conclusion The application of laser suture lysis after trabeculectomy can reduce the early postoperative complications such as shallow anterior chamber and hypotony.
