摘 要:目的评价改良的无悬吊缝线巩膜层间人工晶状体固定术的临床效果。方法对后囊不完整的无晶状体眼35例(35眼)采用改良双针直接经结膜巩膜入路无悬吊缝线的人工晶状体巩膜层间固定术,术后随访6~12个月。结果35例均顺利完成手术。手术前后视力比较,差异有统计学意义(P〈0.05)。手术前后角膜内皮密度、泪膜破裂时间、SchirmerⅠ试验及眼压比较,差异均无统计学意义(P〉0.05)。术后2周裸眼视力≥0.5者33例,占94.3%。2例(5.7%)术后人工晶状体瞳孔夹持,散瞳后退回。1例(2.8%)玻璃体积血,术后6d积血吸收。1例(2.8%)囊样黄斑水肿,术后10d消退。术后未出现人工晶状体移位、人工晶状体襻暴露、严重的葡萄膜反应、继发性青光眼、角膜失代偿、视网膜脱离或脉络膜爆发性出血等并发症。结论改良无悬吊缝线巩膜层间人工晶状体固定术治疗后囊不完整的无晶状体眼是一种简单、微创、安全有效的手术方法。Objective To evaluate the efficacy of modified sutureless intrascleral intraocular lens fixation. Methods Thirty-five eyes of 35 cases of aphakic eyes without enough capsule for IOL supporting were selected. All cases underwent modified sutureless intrascleral intraoeular lens fixation by double needle directly through the conjunctival sclera. The patients were followed up for 6-12 months. Results The operation was successful in all cases. The difference of best corrected visual acuity was significant between before and after operation ( P 〈 0.05 ). The difference was not significant in corneal endothelial density, tear break-up time, Schirmer I text and intraocular pressure between before and after operation ( P 〉 0.05 ). The uncorrected visual acuity≥0.5 at 2 weeks after operation was in 33 cases (94.3%). Intraocular lens pupil capture occurred in 2 cases ( 5.7% ) , the IOLs were returned after mydriasis. Vitreous hemorrhage absorbed in 1 case (2.8%) 6 days after surgery. The cystoid macular edema faded in 1 case (2.8%) 10 days after surgery. No intraocular len displacement, intraocular len loop exposure, severe uveal reaction, secondary glaucoma, corneal decompensation, retinal detachment or explosive choroidal hemorrhage occurred. Conclusion Modified sutureless intrascleral intraocular lens fixation is a simple, minimally invasive, safe and effective method.
