双切口超声乳化联合小梁切除术治疗急性闭角青光眼伴白内障的效果

摘　　要：目的评价双切口晶状体超声乳化联合小梁切除术治疗急性闭角型青光眼伴白内障的临床效果。方法2014年1月至2015年9月急性闭角青光眼伴白内障100例，均施行晶状体超声乳化人工晶状体植入联合小梁切除术，随机分为两组，每组50例。对照组采用单切口手术；观察组采取双切口手术。观察两组患者术后视力、眼压、滤过泡情况和并发症。结果两组患者术后1个月视力以及眼压均较术前提高，观察组优于对照组，差异有统计学意义（P=0．002）；观察组与对照组功能型滤过泡所占比值分别为96．0％和78．0％，差异有统计学意义（P=0．016）。结论双切口超声乳化人工晶状体植入联合小梁切除术治疗急性闭角型青光眼伴发白内障的效果较好。Objective To evaluate clinical efficacy of the two-incisions phacoemulsilication combined with trabeculectomy for the treatment of acute angle-closure glaucoma with cataract. Methods One hundred eyes of 100 cases of acute angle-closure glaucoma with cataract from Jan. 2014 to Sep. 2015 received phacoemulsilication combined with trabeculectomy. All cases were randomly divided into observation group and control group, 50 cases in each group. The control group were treated with surgery with single incision and the observation group with two-incisions. Visual acuity, intraocular pressure, bleb situation and complication after treatment in both groups were observed. Results The visual acuity and intraocular pressure at 1 month after surgery of two groups improved, the observation group was significantly better than the control group, the difference was statistically significant between the two groups （P = 0.002）. Functional filtration bleb of the observation group occupied for 96. 0% and 78. 0% in the observation group. The difference was significant between the two groups （ P = 0. 016 ）. Conclusion The efficacy of double incision phacoemulsifieation combined with trabeculeetomy is better than that of single incision in the treatment of acute angle-closure glaucoma complicated with cataract.
