肝动脉化疗栓塞联合粒子植入治疗肝癌自发性破裂出血的临床疗效

摘　　要：目的:分析肝癌自发性破裂出血联合应用肝动脉化疗栓塞和粒子植入治疗的疗效及安全性。方法:选取66例2014年6月~2017年5月某院收治的肝癌自发性破裂出血患者,分为两组,对照组给予急症TAE术+TACE序贯治疗,研究组给予急症TAE术+粒子植入+TACE序贯治疗,比较两组治疗效果。结果:研究组有效率为93.94%,对照组有效率为72.73%,差异有统计学意义(P<0.05);研究组治疗后6、12、18、24个月生存率均显著高于对照组,差异明显(P<0.05);研究组不良反应发生率为12.12%,对照组不良反应发生率为18.18%,组间差异显著(P>0.05)。结论:肝动脉化疗栓塞和粒子植入联合应用,能够有效降低患者的血清AFP浓度,提高临床效果,延长患者生存期,且具有较高安全性。Objective：To analyze the efficacy and safety of shepatic arterial chemoembolization combined with particle implantation in the treatment of spontaneous rupture and hemorrhage of hepatocellular carcinoma.Methods：66 cases of patients with spontaneous rupture and hemorrhage of liver cancer treated in a hospital from June 2014 to May 2017 were selected and divided into two groups.The control group was given emergency surgery TACE sequential therapy after TAE operation,and the study group was given emergency TAE＋particle implantation ＋TACE sequential therapy,then the treatment effect of the two groups were compared.Results：The effective rate was 93.94%in the study group and 72.73%in the control group,and the difference was statistically significant（P〈0.05）.The survival rate of study group at 6,12,18,24 months after treatment was significantly higher than that of control group,with significant difference（P〈0.05）.The incidence of adverse reactions was 12.12%in the study group and 18.18%in the control group,with significant difference between the two groups（P〉0.05）.Conclusion：The combination of hepatic arterial chemoembolization and particle implantation can effectively reduce the serum AFP concentration,improve the clinical effect,and prolong the survival time of the patients,which has high safety.
