非小细胞肺癌术后免疫功能与铁蛋白、癌胚抗原的相关性

摘　　要：目的观察非小细胞肺癌术后免疫功能与血清铁蛋白(SF)、癌胚抗原(CEA)的相关性。方法回顾性纳入2016年1月至2018年1月收治的90例经手术治疗的非小细胞肺癌为对象。统计患者一般资料、病理学资料,于术后3~4周抽取患者清晨空腹外周静脉血5 ml,采用电化学发光免疫法检测SF、CEA。参照各试剂盒,明确SF与CEA阳性界值,分为四组,即SF阳性组(SF≥336.2 ng/ml,31例)、SF阴性组(SF<336.2 ng/ml,59例)、CEA阳性组(CEA≥5.0ng/ml,42例)、CEA阴性组(CEA<5.0 ng/ml,48例)。采用流式细胞术检测SF阳性组与阴性组、CEA阳性组与阴性组的B细胞、NK细胞、T细胞亚群,并分析SF与CEA的相关性及各自与免疫功能指标的相关性。结果 SF阴性组CEA、B细胞水平与SF阳性组比较差异无统计学意义(P>0.05),SF水平及CD3^+、CD8^+百分比显著低于SF阳性组(P<0.05),NK细胞、CD4^+百分比及CD4^+/CD8^+显著高于SF阳性组(P<0.05)。CEA阴性组B细胞、CD3^+百分比与CEA阳性组比较差异无统计学意义(P>0.05),CEA、SF水平及CD8^+百分比显著低于CEA阳性组(P<0.05),NK细胞、CD4^+百分比及CD4^+/CD8^+显著高于CEA阳性组(P<0.05)。90例非小细胞肺癌患者中,术后CEA水平与NK细胞、T细胞亚群(CD4^+、CD8^+百分比及CD4^+/CD8^+)具有相关性(P<0.05),术后SF水平与NK细胞、T细胞亚群(CD3^+、CD4^+、CD8^+百分比及CD4^+/CD8^+)具有相关性(P<0.05)。结论非小细胞肺癌患者术后出现免疫状态失衡与SF、CEA水平有关,临床上应引起足够重视。Objective To observe the correlation between immune function and serum ferritin（ SF） and Carcino-embryonic antigen（ CEA） in non small cell lung cancer. Methods From January 2016 to January 2018 in our hospital 90 cases of surgically treated non-small cell lung cancer patients were selected as the study object. General information,pathology were recorded. 5 ml peripheral venous blood of morning fasting patients after 3 - 4 weeks was extracted,the electrochemiluminescence immunoassay was used for detection of SF and CEA. According to the kit,SF and CEA positive boundary value were defined and the objects were divided into four groups： SF positive group（ SF≥336. 2 ng/ml,31 cases） and SF negative group（ SF 336. 2 ng/ml,59 cases）,CEA positive group（ 42 cases CEA≥5. 0 ng/ml,CEA） and negative group（ CEA 5. 0 ng/ml,48 cases）. The correlation between SF and CEA was analyzed,and flow cytometry was used to detect B cells,NK cells and T cell subgroups,and the correlation between the four groups was observed. Results There was no significant difference in CEA and B cell level between SF negative group and SF positive group（ P〉0. 05）. SF level and CD3^＋and CD8^＋percentages were significantly lower than those in SF positive group（ P〈0. 05）,and the percentage of NK cells,the percentage of SF and the ratio of them were significantly higher than those in the positive group（ P〈0. 05）. There was no significant difference between CEA negative group of B cells,the percentage of CD3^＋and CEA positive group（ P〉0. 05）,CEA,SF level and the percentage of CD8^＋was significantly lower than that in CEA positive group（ P〈0. 05）,NK,CD4^＋cell percentage and CD4^＋/CD8^＋ratio was significantly higher in CEA positive group（ P〈0. 05）.
