胰腺囊腺瘤和囊腺癌166例临床诊治分析

摘　　要：目的:探讨胰腺囊腺瘤及囊腺癌的临床诊断及治疗方法。方法:收治胰腺囊腺瘤及囊腺癌患者166例,回顾性分析其临床资料及诊治情况。结果:46.99%(78/166)的患者是经CT或B超等影像学检查发现。9例没有行手术治疗,65例接受胰十二指肠切除术,59例接受远端胰腺切除术,9例接受胰腺节段切除术,24例接受肿瘤摘除术。患者术前糖链抗原19-9(CA19-9)单项检测指标的阳性率和糖链抗原125(CA125)单项检测指标的阳性率相比显著性较高。结论:CT和B超是较为主要的胰腺囊性肿瘤检查手段,大多数浆液性囊腺瘤及黏液性囊性肿瘤都要进行手术治疗,在影像学检查基础上可以结合免疫学肿瘤指标检测手段,尤其是对CA19-9的检测可以作为术前判断的重要指标。Objective：To explore the clinical diagnosis and treatment of pancreatic cystadenoma and cystadenocarcinoma.Methods：166 patients with pancreatic cystadenoma and cystadenocarcinoma were selected,and the clinical data and diagnosis and treatment were analyzed retrospectively.Results：46.99%（78/166） patients were found by CT or B-ultrasound.9 cases were not treated with surgical treatment,65 cases underwent pancreaticoduodenectomy.59 cases underwent distal pancreatectomy.9 cases underwent segmental resection of pancreas.24 cases underwent tumor resection.Compared with the positive rate of single detection index of carbohydrate antigen 125（CA125）,the positive rate of preoperatively single indicator of glycan chain antigen19-9（CA19-9） was significantly higher.Conclusion：CT and B ultrasound were the most important methods for the examination of pancreatic cystic tumors.Most serous cystadenoma and mucous cystic tumor need surgical treatment.On the basis of imaging examination,we can combine immunological tumor markers detection methods,especially for the detection of CA19-9,which can be used as an important indicator of preoperative judgement.
