全麻下透明冠在婴幼儿龋前牙修复中的应用

摘　　要：目的:观察全麻下使用透明成形冠在婴幼儿龋前牙复合树脂全冠修复中的临床效果,并与传统直接树脂修复进行比较。方法:回顾性分析在我院接受治疗的56例ECC患儿(248颗乳前牙)临床资料,其中实验组146颗牙,采用透明成形冠技术进行复合树脂全冠修复;对照组102颗牙,采用传统前牙直接树脂充填治疗。分别于治疗后6、18个月进行复查,并从修复体保持、边缘着色、边缘密合性以及继发龋发生情况等方面比较两组修复效果。结果:实验组的修复体保持、边缘着色以及继发龋方面与对照组成功率无统计学差异(P>0.05);而边缘密合性方面,实验组成功率高于对照组,差异有统计学意义(P<0.05)。随访18个月后发现,透明成形冠与复合树脂结合各项指标修复成功率显著高于直接树脂充填(P<0.01)。结论:全麻下治疗婴幼儿龋,使用透明成形冠技术能明显提高修复体的成功率,是比较理想且值得推广的修复方式。Objective To observe the clinical effects of transparent strip crowns in the repair of anterior teeth with composite resin crown under general anesthesia,and compare it with the traditional direct resin restoration.Methods A retrospective analysis was performed based on the clinical data collected from 56 cases（ 248 deciduous front teeth） in our hospital. Total146 teeth in the experimental group were restored with transparent strip crowns,and 102 teeth in the control group were restored with direct resin filling. Patients were followed-up 6 months and 18 months after treatment respectively.The restoration effects of the two groups were compared in the aspects of prosthesis maintenance,marginal pigmentation,edge sealing and secondary caries. Results There was no significant difference（ P〉0.05） between the two groups in the aspects of prosthesis maintenance,marginal pigmentation and secondary caries; but the edge sealing success rate of experimental group was higher than the control group,and the difference was statistically significant（ P〈0.05）. After 18 months of follow-up,the restoration success rate of transparent strip crown was significantly higher than that of direct resin filling（ P〈0.05）.Conclusion Under general anesthesia,the use of transparent strip crowns technology can significantly improve the achievement rate of restoration. It is ideal and worthy of promotion in the clinical application.
