经蝶鼻内镜垂体瘤切除术的治疗体会

摘　　要：目的分析经蝶窦鼻内镜下垂体瘤切除术的疗效和临床意义。方法回顾性分析我科2009年10月至2017年3月间行经蝶鼻内镜手术的31例垂体瘤患者的临床资料,了解手术效果、术后并发症及术后患者症状的改善情况等。结果 31例垂体瘤患者中,29例(93.5%)患者肿瘤被完全切除。术前视力障碍15例患者,6例(40%)术后患者视力、视野改善率。术前垂体功能减退12例患者,6例(50%)术后患者垂体功能改善。术后脑脊液鼻漏1例、尿崩症1例、颅内感染3例经对症治疗后均痊愈;本组无死亡及等严重并发症。结论经蝶鼻内镜下垂体瘤全切除术的一种好术式,并且创伤小,值得临床推广。Objective To analyze the therapeutic effect of transsphenoidal endoscopic sinus surgery for pituitary adenomas and summarize its clinical application value.Methods The data regarding the clinical information,operative effect,postoperative complications,and the improvement of the symptoms of 31 patients with pituitary adenoma underwent transsphenoidal endoscopic surgery from October 2009 to March 2017 in our department were analyzed retrospectively.Results Of the 31 patients with pituitary adenoma,the tumor of the 29（93.5%）patients were completely resected.Of the 15 patients with preoperative visual impairment,6（40%）patients＇ vision and visual field were improved after operation.Of the 12 patients with hypopituitarism before operation,6 patients＇ pituitary function were improved after operation.The 1 patient withcerebrospinal rhinorrhea,1 patient with diabetes insipidus,and 3 patents with intracranial infection were cured after symptomatic treatment.No death case and other severe medical condition were found in this study.Conclusion Transsphenoidal endoscopic pituitary adenectomy is a good surgical procedure with less traumaand worthy of being widely applied in clinic.
