内观认知治疗对精神分裂症康复作用的探讨

摘　　要：目的探讨内观认知治疗对精神分裂症康复期患者的疗效及其对社会功能的影响。方法选取92例单一服用利培酮的住院精神分裂症康复期患者,随机分为治疗组(46例)和对照组(46)例,治疗组予药物治疗同时开展内观认知治疗,对照组则继续予药物治疗。治疗观察期为3个月。在治疗前及治疗后第12周分别以阳性及阴性症状量表(PANSS)、住院精神病人社会功能评定量表(SSPFI)、自知力、治疗态度问卷(ITAQ)量表进行精神症状、社会功能及自知力恢复情况的疗效评定。结果实际完成88例,脱落4例,研究组治疗后PANSS、SSPF、ITAQ总分值及各因子分比较治疗前差异有显著性(P<0.01);研究组和对照组相比较,研究组治疗后PANSS、SSPF、ITAQ评分值较对照组比较有统计学差异(P<0.01)。结论内观认知治疗能有效改善精神分裂症患者的社会功能和精神症状,疗效优于单纯药物治疗,有助于促进精神分裂症患者的康复和预后。Objective To obverse the efficacy and social functional effect of Nai-kan cognitive therapy in treating schizophrenia patients in recovery stage. Methods In this 3-month study, 92 schizophrenia patients in recovery treated with Risperidone alone were randomly assigned to treatment with Risperidone plus Nai-kan therapy （n=46） or Risperidone alone （ n=46 ） . The efficacy of psychiatric symptoms, social function and insight improvement were assessed by the Positive and Negative Syndrome Scale （PANSS）, Scale of Social function for psychosis in-patients （ SSPF ） and the Insight and Treatment Attitude Questionnaire （ ITAQ ） at week 12 before and after treatment, respectively. Results At the end of the study, 88 cases completed and 4 dropped. Compared with pre-treatment, the total and factor scores of PANSS, SSPF and ITAQ in the study group were significantly improved （P 〈 0.01 ） . Compared with the control group, PANSS, SSPF and ITAQ scores in the study group were significantly improved （P〈0.01） . Conclusion Nai-kan cognitive therapy improves the social function and psychiatric symptoms of schizophrenic patients effectively. Nai-kan add on therapy is superior to monotherapy and beneficial to the recovery and prognosis of schizophrenia patients.
