摘  要:目的乳腺癌发病率已占据女性恶性肿瘤之首,但发病机制至今尚未明确。本研究探讨分子标志物Ki-67、人表皮生长因子受体2(human epidermal growth factor receptor-2,HER2)和磷酸化组蛋白H3(phospho-histone H3,PHH3)对预测乳腺癌新辅助化疗疗效的相关性。方法选取2015-01-01-2016-12-31广饶县人民医院接受新辅助化疗的乳腺癌患者110例作为研究对象,通过免疫组织化学方法对瘤体组织化疗前后分子标志物Ki-67、HER2和PHH3进行检测,通过统计学分析化疗前后各因子表达变化以及各因子表达变化的相关性。结果化疗前后有效组Ki-67、HER2和PHH3高表达与无效组比较,差异均无统计学意义,均P>0.05。化疗后Ki-67、HER2高表达明显低于化疗前,而PHH3高表达变化在化疗前后差异无统计学意义。Ki-67、HER2和PHH3表达与年龄、病理组织分型无相关性(P>0.05),与肿瘤分期有相关性(P<0.05)。化疗前后Ki-67与HER2、PHH3表达均呈两两正相关,P<0.01。结论Ki-67、HER2和PHH3在化疗前可能不具有预测新辅助化疗治疗乳腺癌的效果,但可作为评价化疗疗效的指标。OBJECTIVE To investigate the effects of Ki-67,HER2,PHH3 to predict the effect of neoadjuvant chemotherapy on breast cancer in advanced stage.METHODS Expressions of Ki-67,HER2 and PHH3 in breast cancer in advanced stage of 110 patients received neoadjuvant chemotherapy were detected by immunohistochemistry before and after chemotherapy.The relationships of expression levels and effective rate were evaluated.RESULTS There were no differences of Ki-67,HER2 and PHH3 expressions before and after chemotherapy.The positive expressions of Ki-67 and HER2 after chemotherapy were significantly lower than those before chemotherapy,the difference of the expression of PHH3 was not statistically significant(P〉0.05).The expressions of Ki-67,HER2 and PHH3 were significantly associated with clinical stage(P〈0.05),but were not significantly associated with patients age and tumor type(P〉0.05).There were positive correlations in Ki-67,HER2 and PHH3 before and after chemotherapy,P〈0.01.CONCLUSION Ki-67,HER2,PHH3 can't predict the effect of neoadjuvant chemotherapy on breast cancer in advanced stage,but they can be the targets to evaluate the chemotherapy effect.
