口膜控法上颌窦内提升术的临床应用研究

摘　　要：目的:通过临床对照试验研究膜控法上颌窦底内提升术的临床效果。方法:选取40例患者采用膜控法上颌窦内提升或外提升并植入骨替代品,同期或择期植入种植体(上颌窦底内提升组同期植入植体,外提升组择期植入植体)。术后采用视觉模拟量表(VAS)、语言评价量表(VRS)得分分析内外提升术后反应的差异性。并在修复取模型时测量ISQ值,比较两组结果有无统计学差异。结果:膜控法上颌窦提升中有1例穿孔,术中利用可吸收生物膜修补,最终成功修复,经过近2年的随访,患者反应临床效果良好。采用VAS(视觉模拟量表)、语言评价量表VRS的得分分析内外提升患者术后反应的结果差异有统计学意义(P<0.05),二者修复取模型时的ISQ值比较无统计学意义。结论:应用膜控法上颌窦内提升同期植入植体,手术操作相对简便,术中创伤小,术后反应小,并发症少,患者满意度高,值得临床推广应用。Objective：To evaluate the clinical effect of maxillary sinus floor elevation through controlling the tension of maxillary sinus mucosa.Methods：A total of 40 patients received the maxillary sinus floor elevation through controlling the tension of maxillary sinus mucosa from the top of alveolar or lateral window of the maxillary sinus,and the bone substitute material was placed instantly.The implants were placed simultaneously or in other time.The patients were analyzed by cone-beam computed tomography and evaluated by Visual Analogue Scale（VAS）and Verbal Rating Scale（VRS）.The ISQ scales were compared between two groups.Results：Penetrating of sinus mucosa was observed in one case,but the case was successfully treated with absorbable collagen membrane（BioGide）.There was no complaining during two years following up.Significant statistic difference was found in the VAS and VRS（P〈0.05）,however,not in the ISQ scale between two groups.Conclusion：Through controlling the tension of maxillary sinus mucosa,the maxillary sinus floor elevation was predictably simple and minimally invasive,and the satisfactory rate of patients was high.
