摘 要:目的:研究血清癌胚抗原(CEA)、细胞角蛋白片段19抗原21-1(CYFRA21-1)及胸苷激酶1(TK1)在非小细胞肺癌早期诊断中的价值,为临床诊断提供参考。方法:回顾性分析我院2016年1月-2018年1月期间收治的104例非小细胞肺癌患者的临床资料作为观察组,另选取同期来院参加体检的健康志愿者67例作为对照组。对比两组受检者的血清CEA、CYFRA21-1及TK1水平,并分析其表达水平对诊断及联合诊断特异度及灵敏度。结果:两组间相比,非小细胞肺癌患者的血清CEA、CYFRA21-1及TK1水平明显高于对照组,差异有统计学意义(P<0.05)。非小细胞肺癌患者的血清CEA、CYFRA21-1及TK1阳性率分别为48.1%、36.5%、67.3%,均高于对照组,差异有统计学意义(P<0.05),其中对肺鳞癌、肺腺癌敏感性最高的标志物分别为CYFRA21-1(78.3%)及CEA(75.9%)。诊断效能对比,CEA、CYFRA21-1及TK1三者联合检测的灵敏度最高,可达到80.8%。结论:非小细胞肺癌患者的血清CEA、CYFRA21-1及TK1水平及阳性率明显升高,其中CYFRA21-1对诊断肺鳞癌、CEA对诊断肺腺癌敏感性较好,且三者联合检测诊断灵敏度最高,临床上可考虑采用联合检测诊断提高灵敏度。Objective: To study the value of serum carcinoembryonic antigen (CEA), cytokeratin fragment 19 anti- gen 21-1 (CYFRA21-1 ) and thymidine kinase 1 (TKl) in the early diagnosis of non-small cell lung cancer, st) as to provide references for clinical diagnosis. Methods: the clinical data of 104 patients with non-small cell lung cancer who were admitted to our hospital in January 2016 -2018 January were retrospectively analyzed, and 67 healthy volunteers in the same period were selected as control group, The serum levels of CEA, CYFRA21-1 and TKI were compared between the three groups, and the specificity and sensitivity of the diagnosis and joint diagnosis were analyzed. Results: com- pared with the two groups, serum CEA, CYFRA21-1 and TK1 levels in patients with non-small cell lung cancer were significantly higher than those in the control group, the difference was statistically significant (P〈0.05). Non small cell lung cancer patients serum CEA, CYFRA21-1 and TK1 positive rates were 48.1%, 36.5%, 67.3%, were higher than the control group, the difference was statistically significant (P〈0.05) , which were CYFRA21-1 of lung squamous cell carcinoma, lung adenoearcinoma marks the highest sensitivity material (78.3%) and CEA (75.9%). The sensitivity of the combined detection of CEA, CYFflA21-1 and TK1 was the highest, and the sensitivity of CEA, CYFIRA21- 1 and TKI was the highest. Conclusion: the patients with non-small cell lung cancer serum CEA, CYFRA21-1 and TKI level and the positive rate increased significantly, the CYFRA21-1 diagnosis of lung squamous cell carcinoma and CEA diag- nosis of lung adenoearcinmna and better sensitivity, combined with the three highest diagnostic sensitivity, clinical diag- nosis can be used combined with improved sensitivity.
