首次IMRT治疗前行MV-CBCT扫描摆位误差的观察分析

摘  要:目的分析首次调强放疗(IMRT)前行兆伏级锥形束CT(MV-CBCT)扫描对于肿瘤摆位误差的影响。方法选择2016年1月至201746月首次接受IMRT治疗的患者98例,全部患者于首次IMRT治疗前均先行MV-CBCT扫描,对比头颈部肿瘤患者、胸部肿瘤患者行MV-CBCT扫描前、后的摆位误差值,观察MV-CBCT扫描后头部肿瘤与胸部肿瘤摆位误差值的差异性及X轴、Y轴、Z轴间误差值的差异性。结果MV-CBCT校正后头颈部肿瘤首次IMRT治疗摆住误差显著缩小,校正前后X轴、Y轴、Z轴摆位误差差异有统计学意义(P〈0.05);MV-CBCT校正后胸部肿瘤首次IMRT治疗摆位误差显著缩小,校正前后X轴、Y轴、Z轴摆位误差差异有统计学意义(P〈0.05);首次IMRT治疗MV-CBCT校正后胸部肿瘤摆位误差高于头颈部肿瘤,差异有统计学意义(P〈0.05);首次IMRT治疗MV-CBCT校正后X轴、Y轴、Z轴间摆位误差,差异有统计学意义(P〈0.05),Y轴误差高于X轴与Z轴。结论MV-CBCT能够有效校正首次IMRT治疗的摆位误差,提高IMRT治疗的精准性。头部肿瘤首次IMRT摆位误差小于胸部肿瘤,Y轴误差高于X轴与Z轴,应于首次IMRT治疗中给予重点关注。Objective To analyze the first intensity-modulated radiotherapy ( Intensity modulated radiation therapy, IMRT ) on megavoltage cone beam CT ( Megavohage-Cone BeamCT, MV-CBCT ) of scanning for tumor position error. Methods 98 cases from January 2016 to June 2017 in our hospital who were treated by IMRT for the first time were taken as the research objects, all patients in the first IMRT before treatment were first MV-CBCT scan, contrast of head and neck cancer patients, patients with chest tumor MV-CBCT scan before and after the setup errors, the difference of error MV-CBCT was observed after scanning the tumor of the head and thoracic tumor position error value difference and X axis, Y axis, Z axis. Results MV-CBCT head and neck tumor IMP, T treatment for the first time correction of setup error was reduced after correction of X axis, Y axis, Z axis position error was statistically significant, P〈0.05. MV-CBCT after correction of thoracic tumor IMP.T the first treatment setup error was reduced after correction of X axis, Y axis, Z axis position error was the difference, P〈0.05. IMRT for the first time in treatment of MV-CBCT after correction of setup error was higher than that of chest tumor of head and neck cancer, a statistically significant difference of P〈0.05. IMRT for the first time in treatment of MV-CBCT after correction of X axis, Y axis, Z axis position error was statistically significant, P〈0.05, Y axis error was higher than that of the X axis and Z axis. Conclusion MV-CBCT can setup error correction in the first IMRT and improve the precision of IMRT treatment .The group data show that the first IMRT tumor of the head position error is less than the chest tumor, Y axis error is higher than that of the X axis and Z axis, and attention should be given to the first IMRT treatment.
