核糖体蛋白L29在牙龈鳞状细胞癌中表达的临床病理意义

摘　　要：目的研究核糖体蛋白L29(RPL29)在人牙龈鳞状细胞癌(GSCC)中的表达及其临床病理意义。方法青岛大学附属医院口腔颌面外科GSCC 50例,收集肿瘤组织、癌旁组织及正常牙龈组织,应用SP免疫组化法检测RPL29蛋白的表达情况,分析RPL29在GSCC的不同临床T分期、病理分级、有无淋巴结转移以及患者的性别、年龄、吸烟史、饮酒史之间表达的差异,探讨其临床病理意义。结果 RPL29在GSCC中的阳性表达率为66.67%,明显高于癌旁组织的19.05%,且在正常牙龈组织中不表达(阳性率0%),其表达有显著差异(P<0.05)。RPL29的阳性表达分别与GSCC的不同临床T分期及病理分级之间在统计学上无明显差异。RPL29在T1、T2、T3+T4中的阳性率分别为81.82%(9/11)、86.67%(26/30)、66.67%(6/9),统计学无明显差异(P>0.05)。高分化GSCC(36例)与较低分化GSCC(14例)中的RPL29阳性率分别为77.78%(28/36)、92.86%(13/14),无明显差异(P>0.05)。有淋巴结转移者RPL29阳性率100%(15/15),无淋巴结转移者阳性率74.29%(26/35),RPL29的表达在患者有无淋巴结转移组间存在明显差异,且差异有统计学意义(P<0.05)。RPL29表达在不同年龄段、不同性别、有无吸烟、饮酒史等独立分组中无显著差异(P>0.05)。结论 RPL29在牙龈癌中高表达,其表达水平与GSCC患者有无淋巴结转移之间差异显著,对于牙龈癌的转移及预后判断有重要临床病理意义。Objective To study the expression of ribosomal protein L29 in gingival squamous cell carcinoma（ GSCC） and investigate clinical pathological significance of RPL29 in GSCC. Methods Expressions of RPL29 in cancer tissue,paracancerous tissue and normal gingival tissue were detected by SP immunohistochemistry in 50 cases with GSCC. The different expressions of RPL29 and GSCC clinical pathological features,such as clinical T stage,pathological grading,lymph node metastasis,gender,age,smoking and drinking history and so on,were evaluated. Results The expression of RPL29（ 66.67%） in GSCC was higher than that in paracancerous tissue（ 19.05%） and normal gingival tissue（ 0%）（ P〈0.05）. Expression level of RPL29 was not significantly different from clinical T staging and pathological grading of GSCC. The positive expressions of RPL29 in T1,T2,T3＋T4 were respectively 81.82%（ 9/11）,86.67%（ 26/30）,66.67%（ 6/9）（ P〈0.05）. The positive expressions of RPL29 in high differentiation GSCC（ 36 cases） and low differentiation GSCC（ 14 cases） were 77. 78%（ 28/36） and 92. 86%（ 13/14）（ P〉0. 05）. Positive expression of RPL29 was 100%（ 15/15） in patients with lymph node metastasis and 74.29%（ 26/35） in patients without lymph node metastasis（ P〈0.05）. There was no significant difference between expression of RPL29 and age,gender,smoking history,drinking history（ P〉0.05）. Conclusion Expression level of RPL29 is related to lymph node metastasis of GSCC. There is great clinical pathological significance in malignant degree and prognosis of gingival squamous cell carcinoma.
