摘  要:目的比较机械按压装置与救援后徒手胸外按压在淹溺所致心搏骤停患者中的心肺复苏(cardiopulmonaryresuscitation, CPR)效果。方法采用实验对照方法,由14名救生员依次实施4组CPR,即基线组、徒手胸外按压组、Lucas组和MCC组,使用Laerdal计算机系统记录各组之间CPR质量指标,使用视觉模拟评分法测量救生员的自信心和疲劳度。结果MCC组的cc优于徒手胸外按压组,CR2优于基线组,差异有统计学意义(P〈O.05)。但Lucas组和MCC组未达到指南推荐的cD,且Lucas组CR1仅为99.27次/min。徒手胸外按压组疲劳度最高,与MCC组和基线组相比,差异有统计学意义(P〈0.05)。结论MCC在淹溺所致心搏骤停岸上CPR中,较救援后徒手胸外按压在稳定性方面具有明显优势,可对MCC的复苏效果进行进一步研究,以期为淹溺后心搏骤停患者的CPR提供新的选择。Objective To compare the effect of mechanical CPR and manual CPR on cardiac arrest after drowning. Method A quasi-experimental trial of 14 lifeguards was conducted in an indoor pool. The lifeguards performed a baseline CPR, ashore CPR in 3 different situations (manual CPR, Lucas CPR and MCC CPR) after water rescue (100 meters). The quality of CPR, lifeguards' confidence and physical fatigue were observed. Results In the intragroup analysis for CPR quality, we found that MCC CPR group was better than manual CPR group in terms of chest compression and better than baseline group in terms of full chest recoil. Regarding the compression depth, CPR with Lucas and MCC were lower than the recommended depth. The mean compression rate with Lucas was 99.27 per minute. The physical fatigue was highest in manual CPR group, and the difference was significant when compared with MCC and baseline group. Conclusions MCC CPR is superior than manual CPR and Lucas CPR in ashore CPR. Further study is needed and MCC may provide a new option for cardiopulmonary resuscitation after drowning.
