miR-204在原发性肝细胞癌中的表达及评价

摘　　要：目的探讨mi R-204在原发性肝细胞癌(HCC)中的表达及其临床意义。方法采用对照研究,收集2010年5月-2014年5月深圳市宝安区人民医院HCC患者的HCC肝组织36例,切除癌组织后离切缘3~5 cm位置处肝组织作为癌旁组织36例,正常肝组织15例。采用免疫组化法检测mi R-204、B淋巴细胞瘤-2蛋白(Bcl-2)、甲胎蛋白(AFP)、高尔基蛋白73(GP73)、E盒结合锌指蛋白2(ZEB2)、血管内皮生长因子-D(VEGF-D)的表达情况,收集患者临床资料并进行3年随访,分析mi R-204表达情况与患者病理特征、3年生存率的关系,对比检测结果,并使用ROC曲线对检测结果进行评价。结果 36例HCC患者肝癌组织中mi R-204阳性表达12例,阳性表达率33.33%,癌旁组织中mi R-204阳性表达30例,阳性表达率83.33%,正常肝脏组织中mi R-204阳性表达14例,阳性表达率93.33%,肝癌组织mi R-204阳性表达率明显低于癌旁组织及正常组织(P<0.05);肝癌组织的Bcl-2、AFP、GP73、ZEB2、VEGF-D表达阳性率明显高于癌旁组织及正常组织(P<0.05)。HCC患者中的mi R-204表达与Bcl-2、AFP、GP73、ZEB2、VEGF-D呈负相关(r分别为-0.853、-0.769、-0.824、-0.617、-0.732,P=0.000),与HCC病理分期呈负相关(r=-0.794,P=0.000),与HCC患者3年生存率呈正相关(r=0.608,P=0.000)。mi R-204检测的ROC曲线下面积分别为0.945,大于0.6(P<0.05)。结论 HCC患者mi R-204表达降低,对其进行监测,能为其诊断及病情评估提供参考。Objective To investigate the expression of mi R-204 in primary hepatocellular carcinoma（HCC）and its clinical significance.Methods A controlled study was conducted to collect patients with HCC at the People′s Hospital of Baoan Shenzhen from May 2010 to May 2014;the liver tissue of 36 patients with HCC and the liver tissue located at the margin of3～5 cm from the carcinoma were collected after resection;15 normal liver tissue samples were collected as control.The expression of mi R-204,B-cell lymphoma-2（Bcl-2）,alpha fetoprotein（AFP）,Golgi protein 73（GP73）,E box binding zinc finger protein 2（ZEB2）and vascular endothelial growth factor-D（VEGF-D）were detected by immunohistochemical method,and the clinical data of the patients were collected and the patients were followed up for 3 years.The relationship between the expression of mi R-204 and the pathological features and the 3 years＇ survival rate of the patients was analyzed.The results of the test were compared and the results were evaluated by using ROC curve.Results mi R-204 positive expression was found in 12 cases out of 36 cases of HCC and the positive expression rate was 33.33%;the positive expression of mi R-204 in adjacent tissues was found in 30 out of 36 cases,with a positive expression rate of 83.33%.The positive expression of mi R-204 in normal liver tissues was found in 14 out of 15 cases and the positive expression rate was93.33%.The positive rate of mi R-204 expression in HCC tissues was significantly lower than that in adjacent tissues and normal tissues（P〈0.05）.The positive rates of Bcl-2,AFP,GP73,ZEB2 and VEGF-D in HCC tissues were significantly higher than those in adjacent tissues and normal tissues（P〈0.05）.The expression of mi R-204 was negatively correlated with the expressions of Bcl-2,AFP,GP73,ZEB2 and VEGF-D（r were-0.853,-0.769,-0.824,-0.617 and-0.732,respectively,P=0.000）and was also negatively correlated with HCC staging（r=-0.794,P=0.000）;the expression of mi R-204 was positively correlated w
