摘 要:目的探讨实施增加运动康复训练负荷的方案对脑瘫(CP)患儿GMFM-88评分和相关指标的影响。方法选取2015年11月至2016年8月在本院接受康复训练治疗的32例CP患儿,对所有CP患儿均采用增加运动康复训练负荷的训练方案治疗,共训练治疗6w,在方案实施后采用本院自制量表评价CP患儿康复效果,方案实施前和方案实施6w后采用GMFM-88评价CP患儿的运动能力,在方案实施前和方案实施1、6w后检测CP患儿的运动生理生化学相关指标水平。结果训练方案实施6w后,脑瘫患儿康复训练的总有效率为93.75%;训练方案实施6w后,CP患儿的GMFM-88评分较训练方案实施前明显升高,差异有统计学意义(P<0.01);训练方案实施1w后,CP患儿的血红蛋白、血尿素、血清CK和尿蛋白的水平较训练方案实施前明显降低或升高(P<0.01);训练方案实施6w后,CP患儿的血红蛋白、血尿素、血清CK和尿蛋白的水平较训练方案实施1w后有所回升或下降(P<0.01),血红蛋白、血尿素和尿蛋白的水平与训练方案实施前水平相当(P>0.05),血清CK水平仍与训练方案实施前水平比较(P<0.01)。结论增加运动康复训练负荷方案可明显提高CP患儿的GMFM-88评分,改变患儿的血红蛋白、血尿素、血清CK和尿蛋白水平。Objective To investigate the effect of increasing exercise rehabilitation training load on GMFM- 88 score and related indexes of children with cerebral palsy ( CP ) . Method 32 children with CP, who received rehabilitation training in our hospital from November 2015 to August 2016, were selected. All the children with CP were treated with a training program of increased exercise training load, and a total of 6 weeks were trained. After 6 weeks of implementation of the program, the hospital self-made scale was used to evaluate the rehabilitation effect of children with CP; GMFM-88 was used to evaluate the exercise ability of children with cerebral palsy before and after 6 weeks of implementation of the scheme. The level of physiological and biochemical indexes of children with cCP was detected before, 1 weeks and 6 weeks after the implementation of the scheme. Results After 6 weeks of training program, the total effective rate of the rehabilitation training for children with CP was 93.75%; After 6 weeks of training program, the GMFM-88 score of the children with CP was significantly higher than that before the training program,and the difference was statistically significant ( P〈O.Ol ) ; After 1 week of training, the levels of hemoglobin, blood urea, serum CK and urine protein in children with CP decreased or increased significantly, which compared with those before the training program ( P〈0.01 ) ; After 6 weeks of training, the levels of hemoglobin, blood urea, sernm CK and urine protein in children with CP increased or decreased, which compared with those after 1 week of training ( P〈0.01 ) , The level of hemoglobin, blood urea and urine protein was equal to the level before the training program ( P〉0.05 ) ; The level of serum CK was still significantly different from that before the training program ( P〈0.01 ) . Conclusion The program of increasing exercise rehabilitation training load can significantly improve the GMFM-88 score of children with CP, and change the hemoglobin,
