乳腺癌中EPCR的表达及其预后意义

摘　　要：目的观察EPCR在乳腺癌中的表达,并分析其预后意义。方法采用免疫组化Eli Vision法检测99例非特殊型浸润性乳腺癌组织中EPCR蛋白的表达,并复习相关文献。结果 EPCR在乳腺癌组织中的阳性率为36.4%,其表达与组织学分级、淋巴结转移、HER-2及Ki-67表达呈正相关,与ER、PR表达呈负相关,并在管腔A型中呈低表达,差异有统计学意义(P<0.05);Kaplan-Meier单因素生存分析显示,EPCR阳性组的无瘤生存期(disease free survival,DFS)明显低于EPCR阴性组(P=0.011,Log-rank test=6.391),多因素Cox回归分析结果显示,EPCR在pN>1组的乳腺癌中是独立的预后因素(hazard ratio=6.063,P=0.032)。结论 EPCR在乳腺癌中的表达与组织学高分级、淋巴结转移、HER-2阳性、高Ki-67、低ER/PR及更短的DFS相关,提示乳腺癌的不良预后。Purpose To study the expression and prognostic significance of EPCR in breast cancer. Methods An immunohistochemical study was undertaken for measuring the expression of EPCR in 99 invasive breast carcinomas（no special type）. Results The expression of EPCR was 36. 4% in breast cancer,and the expression of EPCR was positively related to tumor grade,lymph node metastasis,HER-2 and Ki-67,but negatively related to ER and PR. Low expression was associated with the Luminal A subtypes（P〈0. 05）. Kaplan-Meier survival analysis showed disease-free survival was significantly worse with EPCR-positive tumours compared to those with EPCR-negative tumours（P = 0. 011,Log-rank test = 6. 391）. Multivariate Cox regression showed that the prognostic significance of EPCR expression（hazard ratio = 6. 063,P = 0. 032） was independent of other common prognostic factors in the subgroup of p N 1 patients. Conclusion An association is identified between EPCR expression and high tumor grade,lymph node metastasis,positive HER-2 and Ki-67,negative ER/PR,and shorter disease free survival,that means the expression of EPCR is associated with an adverse outcome in breast cancer.
