摘 要:目的探讨TP(顺铂+紫杉醇/脂质体紫杉醇)方案联合阿帕替尼在宫颈癌新辅助化疗中的近期疗效,以及对宫颈癌组织中微血管密度(MVD)表达水平的影响。方法对照组采用TP化疗方案,观察组采用TP方案联合阿帕替尼。对比两组患者的治疗效果,治疗前后宫颈癌组织中MVD表达水平变化及MVD差值与治疗效果的相关性。结果两组患者临床疗效比较,差异无统计学意义(P>0.05);治疗3个周期后,两组表达水平与治疗前相比明显下降(P<0.01),且观察组低于对照组(P<0.01);治疗后,有效组MVD表达水平明显低于本组治疗前及无效组治疗后(P<0.01),将治疗前后的MVD表达水平进行相减,并将其差值与治疗效果进行相关性分析,结果发现,两者存在显著的直线正相关关系(P<0.05)。结论在宫颈癌新辅助化疗中,应用TP方案联合抗肿瘤血管生成药物阿帕替尼有协同作用,能够加强抑制新生血管的增殖;MVD表达水平变化作为评价宫颈癌治疗效果的主要指标,有可能成为抗血管生成药物预测疗效的相关生物标志物。Objective To assess the effect of TP( paclitaxel/paclitaxel liposome + cisplatin) combined with apatinib on the level of MVD expressions in neo-adjuvant chemotherapy of cervical cancer. Methods The control group received TP chemotherapy and the observation group did TP scheme plus apatinib. Comparison of expression change in pre-operative and post-operative MVD was done between the two groups. We assessed the correlation of therapeutic efficacy and MVD expression. Results Expression levels of two groups at the end of 3 months administration reduced significantly compared with the levels before administration. Decrease in MVD expression in the observation group was more than that of control group( P〈0. 01). The MVD level of post-treatment in effective treatment group was lower than that of ineffective group( P〈0. 01). The correlation between the level of MVD expression and therapeutic efficacy showed a positive linear relationship( P〈0. 05). Conclusion Combination treatment with TP scheme and apatinib had a synergistic effect on MVD expression and proliferation of new vessel in tumor tissue in the patients with cervical cancer. The result suggests that change in the MVD expression of tumor tissue might be helpful to evaluating the treatment of cervical cancer.
