摘 要:目的对血管生成素-2(Ang-2)在急性胰腺炎(AP)中的表达及其意义。方法健康SD大鼠40只,随机分为5组:对照组、急性胰腺炎模型组(分为AP6h组、AP12h组、AP24h组、AP48h组),每组8只。AP组分别于建模后6、12、24、48h处死,对照组于第2次注射后24h处死。酶联免疫吸附(ELISA)法检测血清Ang-2、肿瘤坏死因子-α(TNF-α)、白细胞介素(IL-6)水平;提取胰腺组织RNA,实时定量聚合酶链反应(Real-time PCR)测定Ang-2、磷脂酰肌醇-3-激酶(P13K)、蛋白激酶B(PKB、Akt)mRNA表达水平。结果同对照组比较,AP组血清Ang-2水平表达水平明显升高,6h即差异有统计学意义,24~48h达到高峰(P=0.032);胰腺组织Ang-2、PI3K、Akt mRNA表达水平明显升高(P=0.028),与AP发生时间呈正相关;炎性相关因子TNF-α、IL-6水平明显升高(P=0.026、0.011)。6-24hAP组血清Ang-2水平与胰腺Ang-2mRNA水平表达呈正相关(r=0.392,P=0.034);P13K、Akt表达水平与Ang-2mRNA水平呈正相关(r=0.432、0.451,P=0.029),血清TNF-α、IL-6表达水平与胰腺Ang-2mRNA水平呈正相关(r=0.412、0.389,P=0.035)。结论血清Ang-2水平在AP发生早期即出现明显升高,持续48h以上,是协助急性胰腺炎诊断的敏感指标;Ang-2可激活P13K/Akt信号通路,也可能是AP过程中TNF-α及IL-6水平升高的原因之一。Objective To investigate dynamic changes of angiopoietin - 2 ( Ang - 2 ) in acute pancreatitis (AP) and its significance, Methods Forty healthy SD rats were randomly divided into 5 groups: control group, acute pancreatitis model group (AP 6 h group, AP 12 h group, AP 24 h group and AP 48 h group). AP group were killed at 6, 12, 24 and 48 h after modeling. The control group was killed after the second injection of 24 hours. The levels of serum Ang - 2, tumor necrosis factor - α ( TNF - α) , interleukin (IL- 6) were detected by enzyme -linked immuuosorbentassay (ELISA). The expression of phosphatidylinositol -3 -hydroxy kinase (PI3K) and protein kinase B (PKB, Akt) mRNA expression levels were detected by Real - time polymerase chain reaction. Results The serum level of Ang - 2 in the AP group was significantly higher than that in the control group(P =0. 032). The expression of PI3K/Akt mRNA in pancreatic tissue of AP group was significantly higher than that in the control group (P = 0. 028 ) , and the levels of TNF - α and IL - 6 were significantly increased ( P = 0. 026, 0. 011 ). The level of Ang - 2 in 6 - 24 h was positively correlated with Ang - 2 mRNA in pancreas ( r = 0. 392, P = 0. 034). The expression of PI3K and Akt was positively correlated with Ang -2 mRNA level (r = 0. 432, 0. 451, P = 0. 029) and the levels of serum TNF - α and IL - 6 were positively correlated with Ang - 2 mRNA levels in pancreas ( r = 0. 412, 0. 389, P = 0. 035). Conclusion Serum Ang - 2 level of AP groupis significandy higher than that of the control group. Aug- 2 can activate PI3K/Akt signaling pathway and may he one of the reasons of high TNF - α and IL - 6 levels in AP. SerumAng - 2 level deteetionmay provide the basis for early assessment of the severity of acute panereatitis.
