摘 要:为探讨不同基因型地中海贫血孕妇之间体内铁代谢相关指标的差异及与非地贫孕妇之间的差异,我们收集地中海贫血孕妇及正常孕妇早孕期或中孕期外周血标本检测血清铁、血清转铁蛋白受体和血清铁蛋白3种铁代谢指标,运用t检验分析两组之间的差异。进一步将地中海贫血孕妇组分成α和β地中海贫血两组,运用随机设计的单因素方差分析法来比较α地中海贫血组、β地中海贫血组和正常对照组3者之间的差异。结果发现地中海贫血孕妇组和正常对照孕妇组的血清铁、血清转铁蛋白受体和血清铁蛋白3个指标均存在显著性差异,p值分别为0.014、0.041、0.039,均小于0.05。而α地中海贫血组、β地中海贫血组和正常对照组三者之间的3种铁代谢指标比较没有统计学差异,两两比较也未发现统计学差异。这表明地中海贫血孕妇体内储铁量高于正常孕妇体内储铁量,要结合地中海贫血孕妇铁代谢指标测值对该类人群进行孕期补铁指导从而避免体内铁含量过高。The study aimed to explore the differences of iron metabolish indicators in pregnant women of different genotype thalassemia and comparison between different genotype thalassemia women and normal women. We collected peripheral blood samples of early or middle pregnancy in women with thalassemia trait and normal women to detect the serum iron (SI), serum transferrin receptor (sTfR), and serum ferritin (SF) levels. Then we compared the differences between two groups by using t-test. Moreover, the thalassemia women were divided into two groups (α-thalassemia and β-thalassemia) and then the differences among erthalassemia group, β-thalassemia group and the control group were compared by using single factor ANOVA method for random design. The significant differences were found in SI, sTfR, and SF level of pregnant thalassemia women and normal women and the p values were 0.014, 0.041, and 0.039 (〈0.05). While there were no significant statistical differences among three groups of iron metabolism and there were no significant statistical differences between any two groups as well. It indicated the iron load in pregnant women with thalassemia was significantly higher than normal pregnant women without thalassemia. We need to measure the iron metabolism indexes before providing iron supplement to pregnant women to avoid excessive iron content in the body.
