针灸联合大柴胡汤加减治疗胆囊切除后综合征临床研究

摘　　要：目的：探究针灸联合大柴胡汤加减治疗胆囊切除后综合征临床疗效。方法：收集我院收治的胆囊切除后综合征患者70例,随机分为对照组和治疗组,各35例,对照组予外科术后西药消炎抗感染等常规治疗,治疗组在对照组的基础上给予针刺治疗联合大柴胡汤加减应用,两组患者均治疗4周。比较两组患者临床疗效、Oddi括约肌基础压、蠕动波幅、胆总管压力、胆总管的扩张程度及不良反应的发生率。结果：治疗后,治疗组Oddi括约肌基础压、蠕动波幅、胆总管压力、胆总管的扩张程度等改善程度较对照组更为明显,治疗组总有效率为91.4%,对照组总有效率为65.7%,治疗组疗效明显优于对照组（P〈0.05）,差异有统计学意义。结论：针灸联合大柴胡汤加减对胆囊切除后综合征的治疗取得良好的临床疗效,能够显著缓解胆囊切除术后综合征（PCS）的症状,起到疏肝利胆、调畅气机、促进胃肠道消化吸收的作用。Objective：To explore the clinical effect of acupuncture combined with Dachaihu Decoction on the treatment of post-cholecystectomy syndrome. Methods：To select 70 cases of our hospital after cystic resection syndrome patients,randomly divided into control group and treatment group,and 35 cases in the control group were given the surgical postoperative inflammation resistance to infection and other conventional Western medicine treatment,the treatment group on the basis of the control group given acupuncture combined with Dachaihu Decoction,two groups of patients were treated for 4 weeks. At the end of the treatment,the clinical total effectiveness was measured,compared with Oddi sphincter pressure,peristalsis amplitude,the basis of the degree of common bile duct pressure and the expansion of the common bile duct,the clinical efficacy of the two groups was compared,and the degree of dilatation of the bile duct and the incidence of adverse reactions were measured. Results：After treatment,the improvement of the treatment group with Oddi sphincter pressure,peristalsis amplitude,the basis of the degree of common bile duct pressure andthe expansion of the common bile duct were more obvious than the control group. The total effective rate of treatment group was 91.4%,and the total effective rate of control group was 65.7%. The treatment group was obviously better than the control group（P〈0.05）,the difference was statistically significant. Conclusion：Acupuncture combined with Dachaihu Decoction have a good clinical effect on the treatment of cholecystectomy syndrome,it can significantly reduce the symptoms of the syndrome（PCS）after cholecystectomy,have the role of soothing liver and relieving bladder,regulating Qi activity and promoting gastrointestinal digestion and absorption.
