健脾益气通络汤联合针灸、物理疗法对神经运动元病患者吞咽功能、肺功能及生活质量的影响

摘　　要：目的：研究健脾益气通络汤联合针灸、物理疗法对神经运动元病（MND）患者吞咽功能、肺功能及生活质量的影响。方法：将我院收治的76例MND患者随机分为接受健脾益气通络汤联合针灸、物理治疗的联合组（38例）和接受针灸、物理治疗的对照组（38例）,比较两组治疗前后吞咽功能、肺功能和生活质量的变化。结果：治疗前,两组吞咽功能评级、第1秒用力呼气量（FEV1）、用力肺活量（FVC）、用力呼气高峰流量占预计值百分比（PEF pred%）、汉化版世界卫生组织生存质量测定量表（WHO QOL-BREF）评分比较,均无显著差异（P〉0.05）;治疗后,两组MND患者吞咽功能评级均显著改善（P〈0.05）,FEV1%、FVC及PEF pred%均显著升高（P〈0.05）,WHO QOLBREF评分均显著提高（P〈0.05）,且联合组患者吞咽功能评级、FEV1%、FVC、PEF pred%及WHO QOL-BREF评分改善幅度均大于对照组（P〈0.05）;联合组总有效率显著高于对照组（89.47%与65.79%,P〈0.05）;两组MND患者治疗3个月中均无肝、肾功能损害等明显不良反应。结论：健脾益气通络汤联合针灸、物理疗法治疗MND的效果显著,能有效改善患者吞咽功能、肺功能,提升患者生活质量,值得临床推广使用。Objective：To study the effect of Jianpi Yiqi Tongluo Decoction combined with acupuncture and physical therapy on the swallowing function,lung function and quality of life of patients with motor neuron disease（MND）. Methods：76 patients with MND in our hospital were randomly divided the combined group（treated with Jianpi Yiqi Tongluo Decoction combined with acupuncture and physical therapy,38 cases）and the control group（treated with acupuncture and physical therapy,38 cases）. Changes of the swallowing function,lung function and quality of life before and after treatment were compared between the two groups. Results：Before treatment,there was no significant difference between the two groups in the grade of swallowing function,forced expiratory volume in 1 second（FEV1）,forced vital capacity（FVC）,the percentage of peak expiratory flow in predicted value（PEF pred%）and scores of the Chinese version of World Health Organization Quality of Life（WHO QOL-BREF）（P〈0.05）. After treatment,grades of swallowing function in the two groups were significantly improved,and FEV1%,FVC,PEF pred% and WHO QOL-BREF scores were significantly increased（all P〈0.05）. The improvement of above-mentioned indexes in the combined group was greater than that in the control group（P〈0.05）. The total effective rate of treatment in the combined group was significantly higher than that in the control group（89.47% vs 65.79%,P〈0.05）. There were no obvious adverse reactions such as liver or renal function damage in the two group during 3 months of treatment. Conclusion：Jianpi Yiqi Tongluo Decoction combined with acupuncture and physical therapy is markedly effective in the treatment of MND. The regimen can effectively improve the swallowing function,lung function and the quality of life of patients.
