血清基质金属蛋白酶9及其基因C1562T多态性与脑梗死患者颈动脉易损斑块的相关性研究

摘　　要：目的研究脑梗死患者的基质金属蛋白酶9(MMP-9)血清学水平及其基因C1562T多态性与动脉粥样硬化斑块易损性的关系。方法采用彩色多普勒超声诊断仪对118例脑梗死急性期的患者进行颈动脉超声检测,根据有无斑块及斑块的回声特点进行分组。采用酶联免疫吸附法检测患者血清MMP-9水平,采用PCR-限制性片段长度多态性技术检测MMP-9基因C1562T的多态性。对结果进行组间比较并分析其相关性。结果颈动脉斑块呈低回声或等回声的患者有69例(易损斑块组),颈动脉斑块呈高回声的患者有49例(稳定斑块组)。易损斑块组血清MMP-9水平[(367.59±28.34)ng/ml]显著高于稳定斑块组[(286.57±32.23)ng/ml](P<0.01)。两组MMP-9基因C1562T基因型CC、CT、TT的比例和等位基因C、T的频率比较,差异无统计学意义。结论血清MMP-9水平可能与脑梗死患者颈动脉斑块稳定性有关,但其基因C1562T多态性与脑梗死患者颈动脉粥样硬化斑块稳定性无明显相关性。Objective To explore the correlations among the serum levels of matrix metalloproteinase-9（ MMP-9） and its gene C1562 T polymorphism and the stability of carotid arteriosclerotic plaque in the patients with cerebral infarction. Methods 118 patients with acute cerebral infarction were detected by color Doppler ultrasonography,and were divided according to the carotid atherosclerotic plaque stability. The serum level of MMP-9 was measured by enzyme-linked immunosorbent assay,and the MMP-9 gene C1562 T polymorphism was detected by PCR-restriction fragment length polymorphism. The results between the groups were analyzed. Results There were 69 cases with equal-echo or low-echo of carotid plaques（ vulnerable plaque group） and 49 cases with high-echo of carotid plaques（ stable plaque group）. The serum level of MMP-9 in vulnerable plaque group[（ 367. 59 ± 28. 34） ng/ml]was significantly higher than that in stable plaque group[（ 286. 57 ± 32. 23） ng/ml]（ P〈0. 01）. There were no significant differences of the frequencies of genotype CC,CT,TT and allele C,T of MMP-9 gene C1562 T between the vulnerable plaque group and the stable plaque group. Conclusion The serum level of MMP-9 may be correlated with the stability of carotid arteriosclerotic plaque in patients with cerebral infarction,while the MMP-9 gene C1562 T polymorphism may have no correlation with the stability of carotid arteriosclerotic plaque in patients with cerebral infarction.
