奥沙利铂联合卡培他滨与ECF方案治疗进展期胃癌疗效比较

摘　　要：【目的】比较奥沙利铂联合卡培他滨方案与ECF方案（表柔比星＋顺铂＋亚叶酸钙＋氟尿嘧啶）治疗进展期胃癌患者的疗效及毒副反应发生情况。【方法】100例进展期胃癌患者，随机分为两组，50例采用ECF方案治疗（对照组），50例采用奥沙利铂联合卡培他滨方案治疗（观察组），每3周为一治疗周期，3个周期后评价疗效和不良反应。比较两组的疗效及毒副反应发生情况。【结果】观察组总有效率为78％（39／50）高于对照组的70％（35／50），但两组比较差异无显著性（P〉0．05）；观察组毒副作用发生率明显低于对照组，且差异有显著性（P〈0．05）。【结论】奥沙利铂联合卡培他滨方案治疗晚期胃癌的有效率和ECF方案相似，但是患者的耐受性好，住院时间短．不霞反应轻微．大客教能捅过对症处理改善．患者的依从性好、生活盾号较高。值得临床推葬使用。[Objective] To compare the curative effect and toxicity of two adjuvant chemotherapy regimens： oxaliplatin with capecitabine vs ECF regimen （epirubicin ＋ cisplatin ＋ leucovorin ＋ fluorouracil） in the treatment of patients with advanced gastric cancer. [Methods]A total of 100 patients with advanced gastric cancer were randomly divided into the control group who underwent the ECF regimen and the observation group who received oxaliplatin combined with capecitabine. There were 50 patients in each group. The efficacy and side effects of the two regimen treatments between the two groups were compared. [Results]The total effective rate in the observa- tion group was 78~ （39/50）, slightly higher than 70% （35/50） in the control group. However, there was no sig- nificant difference between the two groups （ P〉0.05）. The incidence of toxic side effects was significantly lower in the observation group than in the control group and the difference was significant （ P 〈0.05）.[Conclusion]The efficacy of oxaliplatin combined with capecitabine regimen in the treatment of advanced gastric cancer is similar to that of the ECF regimen. However, the patient＇s tolerance is good, the hospitalization time is short, and the adverse reactions are mild. Patients have good compliance and high quality of life. It is worthy of clinical recommendation.
