腹腔镜全结肠系膜切除术治疗结肠癌的疗效及应激反应观察

摘　　要：目的分析腹腔镜全结肠系膜切除术治疗对结肠癌患者疗效及应激反应的影响。方法回顾性分析2016年1月—2017年12月经本院治疗的结肠癌患者80例临床资料,依据手术方案的不同将其分为对照组与观察组,每组40例。对照组行开腹全结肠系膜切除术治疗,观察组行腹腔镜全结肠系膜切除术治疗。对比两组手术指标、应激反应及并发症发生情况。结果观察组失血量(86.42±11.77)ml、肛门首次排气时间(2.26±1.11)d及住院时间(7.10±1.27)d均较对照组短,差异具有统计学意义(P<0.05);术后,两组NE、E、Cor水平均较术前上升,但观察组上述指标水平[(123.06±15.34)ng/ml、(113.63±12.71)ng/ml、(165.33±20.68)ng/ml]较对照组低,差异具有统计学意义(P<0.05);观察组并发症发生率(5.00%)较对照组(20.00%)低,差异具有统计学意义(P<0.05)。结论腹腔镜全结肠系膜切除术治疗结肠癌疗效确切,且具有失血少、恢复快、应激反应小、并发症少等优势。Objective To analyze the efficacy of laparoscopic complete mesocolon excision（ CME） in treatment of colon cancer and its influence on stress response. Methods The clinical data of patients with colon cancer,those received treatment in our hospital from January 2016 to December 2017,were retrospectively analyzed. All the patients were divided into control group and observation group by surgical regimens,with 40 cases in each group. Control group was given open CME,while observation group was treated with laparoscopic CME. The surgical indexes,stress response and complications of the two groups were compared. Results The bleeding volume（ 86.42±11. 77 ml）,first anal exhaust time（ 2. 26 ± 1. 11 d）,hospital stays（ 7. 10 ± 1. 27 d） in observation group were less or shorter than those in control group（ P〈0. 05）. After surgery,the levels of NE,E and Cor in both two groups increased compared with those before surgery,but the above-mentioned indexes in observation group（ 123.06± 15. 34 ng/ml,113. 63 ± 12. 71 ng/ml,165. 33 ± 20. 68 ng/ml） were lower than those in control group（ P〈0. 05）. The incidence of complications in observation group（ 5. 00%） was lower than control group（ 20. 00%）（ P〈0. 05）. Conclusions The efficacy of laparoscopic CME in treatment of colon cancer is exact,which has advantages of less blood loss,quick recovery,slight stress response and less complications.
