食管癌术后吻合口瘘35例治疗体会

摘　　要：目的探讨食管癌术后并发吻合口瘘原因和预防措施。方法回顾性分析2007年1月—2017年8月食管癌术后发生吻合口瘘的患者临床资料。结果 10例颈部吻合口瘘均治愈出院,25例胸内吻合口瘘治愈21例,4例死亡。结论吻合口瘘是胸内切除食管癌切除术后的严重并发症,也是术后死亡的主要原因。积极有效引流和营养支持是吻合口瘘治疗的关键。Objective To explore the reasons and prevention measures of anastomotic fistula after esophagectomy.Methods Clinical materials of esophageal cancer patients with postoperative anastomotic fistula from January 2007 to August 2017 were analyzed retrospectively.Results 10 cases of cervical anastomotic fistula were cured and discharged,21 cases of chest anastomotic fistula cured and 4 case died.Conclusion Anastomotic leak is the severe complication of esophagectomy and the main cause of postoperative death.Positive and effective drainage and nutrition support are the keys to treatment of anastomotic leak.
