摘　　要：目的探讨老年人群最常关注的6种皮肤良性肿瘤治疗疗效的影响因素。方法对本科自2010年12月-2016年11月治疗的500例常见老年良性皮肤肿瘤临床资料进行回顾性研究。结果 500例皮损位于头面部374例、躯干部42例、肢端84例;手术224例,其中病理发现癌前病变3例,基底细胞癌2例;冷冻104例,CO2激光172例。卡方检验显示,年龄、病程、是否浸润、术前诊断、治疗方法是疗效的影响因素,多因素回归分析显示年龄、病程及治疗方法是肿瘤治疗显效的影响因素。结论皮肤良性肿瘤治疗方法依皮损的特点和性质而定,也要考虑老年患者的身体综合状况和皮肤状态以及耐受性;手术切除覆盖面广最彻底、安全性高、复发率低,病理检验为肿瘤最终诊断;而基底较浅的、较小或处于肢端不易手术部位的皮损可采取冷冻或激光方法,但不易彻底,需反复治疗多次。Objective To explore influencing factors in elderly benign dermatome by different therapies. Methods The clinical data of 500 cases with elderly benign dermatome collected from December 2010 to November 2016 were retrospectively analyzed. Results Of the 500 cases of skin lesions,374 cases were on head or face,42 cases on trunk and 84 cases on acra. In 224 cases of surgical operation,3 cases of precancerous lesion and 2 case of basal cell carcinoma were found,104 cases were frozen,and 172 cases were treated by CO2 laser. Analysis showed that age,duration,infiltration,preoperative diagnosis and treatments were factors that influence the curative effect. Multiple factors Logistic regression analysis showed age,duration and treatments were independent influencing factors. Conclusion Treatment of benign dermatoma depends on the characteristics and nature of skin lesions,but also on the elderly patients＇ physical status,skin condition and tolerance; surgical resection covers the most complete coverage,and has the advantages of high safety,low recurrence rate. Pathological examination is the final diagnosis of tumor; smaller or less surgical sites can be treated by freezing or laser treatment,but not easy to be treated repeatedly.
