术中单侧肺循环短时阻断在局限性晚期非小细胞肺癌中的临床价值研究

摘　　要：目的探讨术中单侧肺循环短时阻断在局限性晚期非小细胞肺癌中的临床价值。方法选择80例局限性晚期非小细胞肺癌患者,按照随机数字表法分为观察组与对照组,每组40例。对照组患者行常规术中肺动脉阻断术,观察组患者行单侧肺循环短时阻断术。比较两组患者的手术时间、术中出血量、术后有创呼吸机支持时间和总住院时间,统计两组患者术后复发和远处转移情况,绘制1年生存曲线。结果观察组患者的手术时间、术后有创呼吸机支持时间、总住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P﹤0.01);观察组患者术后复发率、远处转移率均低于对照组,1年累积生存率高于对照组,差异均有统计学意义(P﹤0.05)。结论局限性晚期非小细胞肺癌的手术治疗,相对于常规肺动脉阻断术,术中单侧肺循环短时阻断可以减少术中出血量,缩短手术时间,降低复发率和远处转移率,提高患者的生存率。Objective To investigate the clinical value of intraoperative unilateral pulmonary circulation short-term block in locally advanced non-small cell lung cancer. Method Eighty patients with locally advanced non-small cell lung cancer were randomly divided into observation group and control group according to the random number table, each group had fourty patients. The patients in control group were treated with routine operation pulmonary artery occlusion,while the patients in observation group underwent unilateral pulmonary circulation obstruction. The operation time, intraoperative bleeding volume, invasive ventilator support time and total hospitalization time of the two groups were compared, and the recurrence（rate） and transfer cases（rate） were calculated, then 1-year survival curve of two groups were drawn. Result The operation time, invasive ventilator support time and total hospitalization time in observation group were significantly shorter than those in control group, and the intraoperative bleeding volume in observation group was lower than that of the control group（P〈0.01）. The recurrence rate and distant metastasis rate of observation group were significantly lower than those of control group（P〈0.05）. The 1-year survival rate of observation group was higher than that of control group（P〈0.05）. Conclusion For the treatment of locally advanced non-small cell lung cancer surgery,compared to pulmonary artery obstruction, intraoperative unilateral pulmonary circulation short-term block can significantly reduce intraoperative bleeding volume, shorten the operation time, reduce the recurrence rate and distant metastasis rate, and increase patient survival rate.
