莱斯特评估包在全科医生门诊接诊能力培训中的应用效果分析

摘　　要：2016年1—12月，以莱斯特评估包（LAP）的培训方式对上海市嘉定区安亭镇黄渡社区卫生服务中心35名全科医生开展社区门诊临床接诊能力培训以及医患交流技能培训；培训1年后评价分析其临床接诊能力在培训前后的差异。结果显示，通过培训，全科医生在临床技能和医患交流方面均有改善。病史采集、临床思维和健康教育3个方面培训后评分均高于培训前；培训前后门诊病史书写和接诊时间评分差别无统计学意义；在建立医患关系、病史采集和反馈、从患者角度理解问题、信息共享和知情同意4个方面，培训后评分和合格率均高于培训前，均（P〈0．05）。提示LAP培训能有效提高全科医生的临床接诊和医患交流技能，对全科医生的培训工作具有借鉴意义。Thirty-five general practitioners （GPs） from Huangdu Subdistrict Community Health Center of Anting Township were trained for clinical skills and communication skills in consultation at outpatient department by using Leichester assessment package （LAP） from January 2016 to December 2016. The clinical skills and communication skills were assessed before and after 1 year of training. The results showed that compared to those before training the clinical skill and communication skill were significantly increased after the training. The scores of medical history taking, clinical reasoning and health education, as well as the scores of building doctor-patient relationship, collecting information and providing feedback, understanding complaints from patient perspective, sharing information and obtaining informed consents after the training were all higher than those before training （ all P 〈 0.05 ）. However, there were no significant differences in scores of history writing and interview time before and after training. The study showed that Leichester assessment package can effectively improve the training of clinical skills and communication skills for GPs, and it can be applied for future training.
