内镜下止血联合质子泵抑制剂泵入在中老年消化性溃疡并发出血中应用

摘　　要：目的探讨内镜下止血联合质子泵抑制剂泵入治疗中老年消化性溃疡并发出血的应用效果。方法选取自2014年10月至2016年12月煤炭总医院收治的100例中老年消化性溃疡并发出血患者为研究对象,采用随机数字表法将患者分为常规组与观察组,每组各50例。常规组在补液、吸氧等治疗基础上接受质子泵抑制剂泵入、止血剂等治疗;观察组在常规组治疗基础上开展内镜下止血治疗。比较两组患者治疗后12 h、24 h及48 h的止血情况、治疗效果以及并发症发生情况。结果观察组患者治疗后12 h、24 h、48 h止血率均高于常规组(P<0.05);观察组治疗有效率为96.0%(48/50),高于常规组的82.0%(41/100),组间比较,差异有统计学意义(P<0.05);观察组并发症发生率为4.0%(2/50),低于常规组的18.0%(9/50),组间比较,差异有统计学意义(P<0.05)。结论内镜下止血联合质子泵抑制剂泵入治疗中老年消化性溃疡并发出血效果显著,能快速止血,调节胃酸分泌,改善胃部内环境,降低再出血风险。Objective To study the effect and value of endoscopic hemostatie combined with proton pump inhibitor in the treatment of peptic ulcer complicated with bleeding. Methods A retrospective study was performed on 100 elderly patients with peptic ulcer complicated with bleeding in our hospital from October 2014 to December 2016. According to the random number table method, the patients were divided into control group and study group ,with 50 cases in each group. The patients in the control group were treated with proton pump inhibitors,and hemostatic agents,while the patients in the study group were treated with endoscopic hemostasis on the basis of the control group. The changes of hemostasis and therapeutic effect were compared between the two groups after 12 hours and 24 hours, 48 hours treatment. Results The hemostatic rate of the study group was significantly higher than that of the control group at 12 hours, 24 hours and 48 hours after treatment（P 〈 0. 05 ）. The total effective rate in the study group was 96. 0% （48/50）, significantly higher than that in the control group（ 82.0% ,48/50 ）. The incidence of complications in the study group was 4.0% （2/50）, significantly lower than that in the control group（ 18.0% , 9/50,P 〈 0. 05 ）. Conclusion Endoscopic hemostasis combined with proton pump inhibitor in the treatment of elderly peptic ulcer complicated by bleeding is effect. It can quickly stop bleeding, regulate gastric acid secretion, decrease the clinical promotion.
