二氧化碳激光切除及焊接皮肤切口治疗面部皮脂腺囊肿的效果

摘　　要：目的探讨二氧化碳激光切除及焊接皮肤切口治疗面部皮脂腺囊肿的临床效果。方法选取2016年1月~2017年6月我院收治的面部皮脂腺囊肿合并感染坏死者80例,采用随机数字表法将其分为对照组、观察组,每组40例。观察组使用武汉光大康医疗设备有限公司提供的KHC-H-1型二氧化碳激光治疗仪,激光治疗波长设定为600~700 nm,光斑直径在窗口100~120 mm,输出功率超过3 W,针对皮脂腺囊肿病灶部位进行针对性照射治疗,每天1次,每次15 min,随后选择皮肤切口使用二氧化碳激光焊接。对照组实施局部麻醉下手术切除治疗,术后联合类人胶原蛋白敷料,比较两组临床症状(红肿消退、创面愈合及囊肿消失)改善时间、皮肤性质测定结果,并统计两组临床治疗效果。结果观察组患者临床症状改善时间均显著早于对照组,差异有统计学意义(P<0.05)。观察组皮肤性质测定测定结果均显著优于对照组,差异有统计学意义(P<0.05)。观察组患者的总有效率显著高于对照组,差异有统计学意义(χ2=19.771,P=0.000<0.05)。结论针对皮脂腺囊肿,使用二氧化碳激光切除及焊接皮肤切口治疗,能够有效提高手术治疗效果,减少术后瘢痕形成,提高术后皮肤质量与性质,从而提高整体临床治疗效果。Objective To investigate the clinical efficacy of Carbon Dioxide laser ablation and welding skin incision in the treatment of facial sebaceous cyst.Methods A total of 80 cases of facial sebaceous cyst complicated with infection and necrosis were selected from January 2016 to June 2017 in our hospital,and were divided into control group and observation group by random number table method,with 40 cases in each group.The observation group used KHC-H-1 Carbon Dioxide laser therapeutic apparatus provided by Wuhan Everbright Kang Medical Equipment Co.,Ltd.,the laser treatment wavelength was set to 600-700 nm,the spot diameter was 100-120 mm in the window,and the output power was over 3 W.Targeted irradiation treatment was carried out for sebaceous cyst lesions once a day for 15 min each time,and then skin incision was selected to be welded with carbon dioxide laser.The control group was treated by surgical resection under local anesthesia,and after operation,combined with human-like collagen dressing,the improvement time of clinical symptoms（redness and swelling subsided,wound healing and cyst disappeared） in two groups were compared,the results of skin properties measurement in two groups were measured,and the clinical treatment effects in two groups were statistically analyzed.Results The improvement time of clinical symptoms in the observation group was significantly earlier than that in the control group,and the difference was statistically significant（P〈0.05）.The test results of skin properties in the observation group were significantly better than those in the control group,and the differences were statistically significant（P〈0.05）.The total effective rate of patients in the observation group was significantly higher than that in the control group,and the difference was statistically significant（χ^2=19.771,P=0.0000.05）.Conclusion For sebaceous cysts,the use of Carbon Dioxide laser ablation and welding skin incision treatment can effectively improve the surgical treatment effect,reduce postop
