摘　　要：报告1例滑车上动脉带蒂额部皮瓣联合多种皮瓣修复面中部巨大基底细胞癌术后缺损。患者女,35岁。面部肿物及斑块5年余。皮肤科检查:鼻部,右侧上、下眼睑及内眦可见大片境界较清淡黑色斑块,边缘呈堤状及串珠样隆起,皮损中央可见萎缩性瘢痕、黑色丘疹及斑块。皮损组织病理检查:表皮变薄,真皮中瘤体由基底样细胞组成,核大,胞质少,染色嗜碱,周边细胞呈栅栏状排列,肿瘤团块与周围基质间有收缩间隙,周围少量淋巴细胞浸润。诊断:基底细胞癌。治疗:予手术切除后,行带蒂额部皮瓣联合多种皮瓣修复缺损,美容效果满意。A case of pedicle flap with superior trochlear artery combined with multiple flaps for reconstruction of large de fect after resection of basal cell carcinoma on the face is reported. A 35-year-old female presented with facial masses and plaques for more than 5 years. Physical examination showed a large area of light black papules on the nose, right upper and lower eyelids, and inner eanthus. Lesions appeared elevated borders with central atrophic scars. Histopathologieal examination revealed thin epidermis and dermal tumors composed of basaloid cells with large nuclei and scant basophilie cytoplasm. Tumors were surrounded by palisading cells. There was a contracted gap between tumor masses and the surrounding matrix, surrounded by a small amount of lymphocytic infiltration. Diagnosis of basal cell carcinoma was made. After surgical resection, reconstruction with pedicle flap combined with multiple flaps was performed, and the cosmetic effect was satisfactory.
