腹腔镜辅助下直肠外翻拖出式手术在中低位直肠癌治疗中的无瘤技术效果

摘　　要：目的分析腹腔镜辅助下直肠外翻拖出式手术在中低位直肠癌治疗中的无瘤技术效果。方法选择2015年2月—2017年5月在该院接受治疗的中低位直肠癌患者102例,随机分为参照组和研究组,各51例患者。参照组患者实施直肠癌低位前切除手术进行治疗,研究组患者实施腹腔镜辅助下直肠外翻拖出式手术进行治疗。对两组患者的手术情况以及治疗的效果进行对比分析。结果两组患者的手术时间(t=0.15,P>0.05)、术中出血量(t=0.63,P>0.05)和清扫淋巴数量(t=0.22,P>0.05)差异无统计学意义;研究组患者的病理切缘距离(3.92±1.26)cm明显较参照组患者(2.89±1.18)cm更长,差异有统计学意义(t=2.92,P<0.05);两组患者的复发差异无统计学意义(χ~2=1.02,P>0.05)。结论针对中低位直肠癌患者使用腹腔镜辅助下直肠外翻拖出式手术无瘤技术其治疗效果良好,可以达到无瘤的效果,且手术操作更加简单,安全性较高。Objective To analyze the tumor-free technology effect of laparoscopic assisted rectal eversion pull-through operation in treatment of middle and lower rectal cancer. Methods 102 cases of middle and lower rectal cancer patients treated in our hospital from February 2015 to May 2017 were selected and randomly divided into two groups with 51 cases in each, the control group were treated with anterior resection of low rectal cancer, while the research group were treated with laparoscopic assisted rectal eversion pull-through operation, and the operation situation and treatment effect were compared and analyzed. Results In the operation time, intraoperative bleeding amount and lymph nodes dissection number between the two groups the difference was no statistically significant（t=0.15,P〈0.05）,（t=0.63,P〈0.05）,（t=0.22,P〈0.05）], and the pathological cutting edge distance in the research group was longer than that in the control group, [（3.92±1.26）cm vs（2.89±1.18）cm]the difference was statisticall significant（t=2.92,P〈0.05）, and there were no obvious differences between the two groups（χ~2=1.02,P〈0.05）. Conclusion The tumor-free technology effect of laparoscopic assisted rectal eversion pull-through operation in treatment of middle and lower rectal cancer is good, with simpler operation and higher safety.
