经支气管镜氩离子凝固术治疗中心型肺癌咯血的临床研究

摘　　要：目的探究对中心型肺癌咯血患者行以经支气管镜氩离子凝固术治疗的临床效果。方法择取2016年12月—2017年12月该院收治的66例中心型肺癌咯血患者,随机将所选患者分成对照组和研究组,对照组33例行经支气管镜介入局部用药联合全身化疗,研究组33例行经支气管镜氩离子凝固术治疗,对两组患者的临床效果进行分析和比较。结果研究组临床总有效率为78.79%,对照组为54.55%,差异有统计学意义(χ~2=4.364,P=0.037);研究组发热缓解率、咳嗽缓解率、咯血缓解率、胸痛缓解率、呼吸困难缓解率分别为81.82%、75.76%、90.91%、57.58%、93.94%,对照组分别为48.48%、51.52%、69.70%、33.33%、72.73%,差异有统计学意义(χ~2=8.075、4.191、4.694、3.911、5.346,P=0.004、0.041、0.030、0.048、0.021)。结论对中心型肺癌咯血患者行以经支气管镜氩离子凝固术治疗效果良好,可以有效缓解临床症状,提高肺功能,具有显著临床价值。Objective To explore clinical effect of the bronchoscopic argon plasma coagulation in the treatment of central type lung cancer hemoptysis. Methods 66 cases of patients with center-type lung cancer emptysis admitted and treated in our hospital from December 2016 to December 2017 were selected and randomly divided into two groups with 33 cases in each, the control group used the transbronchial interventional local medication combined with general chemotherapy, while the research group were treated with bronchoscopic argon plasma coagulation, and the clinical effect of the two groups was analyzed and compared. Results The total clinical effective rate in the research group and in the control group was respectively 78.79% vs 54.55% the difference was statistically significant（χ^2=4.364, P=0.037）,and the fever relief rate, cough relief rate and hemoptysis relief rate and chest pain relief rate and dyspnea in the research group and in the control group were respectively 81.82%, 75.76%, 90.91%, 57.58%, 93.94%） vs（48.48%,51.52%, 69.70%, 33.33%, 72.73% the difference was statistically significant（χ^2=8.075, 4.191, 4.694, 3.911, 5.346, P=0.004, 0.041, 0.030, 0.048, 0.021）. Conclusion The effect of the central type of lung cancer in treatment of patients with hemoptysis by bronchoscopic argon plasma coagulation is good, which can effectively relieve the clinical symptoms and improve the lung function, and the clinical value is obvious.
