单双层缝合和吻合器吻合在胃癌根治术中的效果比较

摘　　要：目的探讨单双层缝合以及吻合器吻合在胃癌根治术中的应用效果。方法 81例胃癌根治术患者,随机分为三组各27例。A1组采用单层缝合,A2组采用双层缝合,A3组采用吻合器吻合。比较三组患者的术中术后指标、并发症发生率以及肿瘤转移率。结果 A3组的术中失血量、手术时间均显著少于A1组以及A2组(P<0.05)。A1组、A3组患者的并发症发生率显著低于A2组(P<0.05)。三组的肿瘤远处转移率以及局部转移率比较,差异无统计学意义(P>0.05)。结论临床对胃癌根治术患者采用吻合器吻合,在缩短手术时间、减少术中失血量以及降低并发症发生率方面有显著效果,从而提高手术治疗效果。Objective To explore the application effects of single-layer suture, double-layer suture and stapler anastomosis in radical gastrectomy. Methods 81 cases of patients with radical gastrectomy were randomly divided into three groups, with 27 cases in each group.Group A1 received single-layer suture, group A2 received double-layer suture, and group A3 received stapler anastomosis. The intraoperative and postoperative indicators, incidence of complications and tumor metastasis rate were compared among the three groups. Results The intraoperative blood loss volume and operation time of group A3 were significantly lower than those of group A1 and group A2（P〈0.05）.The incidence of complications of group A2 was significantly higher than that of group A1 and group A3（P〈0.05）. No statistical difference was found among three groups in the tumor distant metastasis rate or tumor local metastasis rate（P〈0.05）. Conclusions Stapler anastomosis applied in patients with radical gastrectomy has significant effects in shortening operation time, reducing intraoperative blood loss volume and lowering the incidence of complications, so to improve the curative effect of surgery.
