摘　　要：目的探讨低位直肠癌保肛手术与腹部人工肛门治疗的临床疗效。方法选取我院(2014年1月至2017年1月)收治的低位直肠癌患者46例为研究对象,根据手术方案差异分为观察组与对照组,每组23例,观察组采用低位直肠癌保肛手术治疗,对照组采用Miles术治疗。观察并记录两组患者的输血率、满意率、手术时间、术中出血量、术后并发症发生情。结果两组患者满意率对比无明显差异,不具有统计学意义(P>0.05);观察组手术时间、术中出血量均优于对照组,差异具有统计学意义(P<0.05);观察组肠道恢复时间明显短于对照组,差异具有统计学意义(P<0.05),两组住院时间对比无明显差异,不具有统计学意义(P>0.05);观察组术后并发症发生率明显低于对照组,差异具有统计学意义(P<0.05)。结论低位直肠癌采用低位直肠癌保肛手术治疗效果比Miles术治疗效果好,具有小创伤、出血量少、肠道恢复时间快、并发症发生率低等优点,值得临床推广。Objective To investigate the clinical efficacy of sphincter preserving surgery and abdominal artificial anus in low rectal cancer. Methods Inour hospital （2014 years 1 months -2017 years 1 months） from 46 patients with low rectal cancer patients as the research object, according to the operationscheme were divided into observation group and control group, 23 cases in each group, the observation group with rectal cancer surgery, the control groupwas treated with Miles. The blood transfusion rate, satisfaction rate, operation time, intraoperative blood loss and postoperative complications of thetwo groups were observed and recorded. Results Two groups of patients with no significant difference between the rate of contrast was not statisticallysignificant （P 〉0.05）; to observe the operative time, bleeding volume were better than the control group, the difference was statistically significant （P 〈0.05）;observation group, intestinal recovery time was significantly shorter than the control group, the difference was statistically significant （P 〈0.05）, no obviousthe differences between the two groups in contrast, was not statistically significant （P 〉0.05）; the incidence of complications was significantly lower than thecontrol group patients in the observation group, the difference was statistically significant （P 〈0.05）. Conclusion Lower rectal cancer with lower rectal cancersphincter preservation surgery treatment effect is better than Miles treatment, with small trauma, less bleeding, intestinal recovery time, low incidence ofcomplications, it is worthy of clinical promotion.
