益生菌联合铋剂四联疗法治疗幽门螺杆菌的疗效分析

摘　　要：目的探讨含铋剂四联方案基础上加用益生菌在幽门螺杆菌(Helicobacterpylori,Hp)根除治疗中的疗效。方法将260例Hp阳性的患者随机分为:A组(含铋剂四联疗法)、B组(A组基础上加用双歧杆菌三联活菌)及C组(A组基础上加用复合乳酸菌),疗程均14 d。疗程结束后复查14C-尿素呼气试验评估根除疗效,对于消化性溃疡患者复查胃镜评估溃疡愈合情况。治疗期间记录患者症状缓解情况、不良反应发生情况。结果 B、C两组Hp根除率和症状缓解率显著高于A组(P<0.05),HP根除率(ITT分析:93.1%、92.7%、83.3%;PP分析:94.2%、93.9%、86.2%,症状缓解率:89.7%、88%、78.8%),B、C组间差异无统计学意义。三种方案的消化性溃疡痊愈率无明显差异84.4%、866%和81%,(P>0.05)。B、C组不良反应发生率显著低于A两组(4.5%、4.8%、19%,P<0.05)。结论在铋剂四联根除疗法基础上加用益生菌的抗Hp方案能减轻患者胃肠道不良反应,提高Hp的根除率。Objective Discussed the effect of probiotics on the basis of bismuth-based quadruple therapy for the eradication of helicobacter pylori. Method The 260 patients with Hp positive were randomly divided into three groups： A groups（Bismuth-based quadruple therapy）, B groups（On the basis of group A, bifidobacterium trigeminae was added）, C groups（On the basis of group A, combine with compound lactic acid bacteria）, all treatment sessions were 14 days. And after the treatment, 14 C-urea exhalation test was performed to evaluate the efficacy of eradication, and review the gastroscope to evaluate the healing of ulcer in the patients with peptic ulcer.Recorded the symptom remission and adverse reactions during the treatment about all patients. Result The Hp eradication rate and symptom remission rate were significantly higher in group B and group C compared with group A（P〈0.05）; but there was no statistically significant difference about HP eradication rate（ITT analysis： 93.1%、92.7%、83.3%; PP analysis： 94.2%、93.9%、86.2%） and symptom remission rate（89.7% vs 88% vs 78.8%） between group B and C. There was no significant difference in the recovery rate of peptic ulcer between three regimens（84.4% vs 86.6% vs 81%, P〈0.05）. Conclusion The Hp eradication regimens of bismuth-based quadruple therapy combined with probiotics can reduced the adverse reactions of gastrointestinal tract and improve the eradication rate of Hp.
