补肾祛瘀中药联合芳香化酶抑制剂治疗绝经后激素受体阳性乳腺癌疗效及对雌激素、骨密度、骨转换指标的影响

摘　　要：目的观察补肾祛瘀中药联合芳香化酶抑制剂治疗绝经后激素受体阳性乳腺癌疗效及对雌激素、骨密度(BMD)、骨转换指标的影响。方法将100例绝经后激素受体阳性乳腺癌患者随机分为2组,对照组50例给予西药治疗,观察组50例在对照组治疗基础上加用补肾祛瘀中药治疗,观察2组治疗前后中医证候积分、Kupperman评分、雌二醇(E_2)、BMD、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)及骨钙素N-端中分子片段(NMID)水平,统计2组临床疗效和不良反应发生情况。结果治疗后2组腰膝酸软、五心烦热、眩晕耳鸣、口燥咽干及潮热盗汗评分,Kupperman评分,E_2、β-CTX及N-MID水平均显著降低(P均<0.05),BMD水平显著提高(P均<0.05),且观察组治疗后以上指标均显著优于对照组(P均<0.05);观察组治疗总有效率显著高于对照组(P<0.05),不良反应发生率显著低于对照组(P<0.05)。结论补肾祛瘀中药联合芳香化酶抑制剂治疗绝经后激素受体阳性乳腺癌可有效改善相关症状体征,降低E_2水平,改善骨密度,调节骨转换指标,且有助于减轻药物不良反应。Objective It is to observe the curative effect of Chinese herbal medicine for tonifying kidney and clearing stagnation combined with aromatase inhibitor on postmenopausal hormone receptor-positive breast cancer and its effects on estrogen,bone mineral density（ BMD） and bone turnover index. Methods 100 cases of postmenopausal hormone receptorpositive breast cancer patients were randomly divided into 2 groups,50 cases of the control group were treated with western medicine,and 50 cases of the observation group were treated with Chinese medicine for tonifying kidney and clearing blood stasis based on the therapy in the control group. TCM syndrome scores,Kupperman scores,the levels of estradiol（ E2）,BMD,type I collagen carboxy terminal peptide β-specific sequence（ β-CTX） and osteocalcin N-terminal molecular fragment（ N-MID） before and after treatment were observed,the clinical efficacy was evaluated and adverse reactions occurred were recorded in the two groups. Results After treatment,the scores of soreness and weakness of waist and knees,dysphoria in chestpalms-soles,dizziness,tinnitus,dry throat and hot flashes,Kupperman score,the levels of E2,β-CTX and NMID were significantly decreased（ P〈0. 05）,BMD level was significantly increased（ P〈0. 05）; and the improvements of above indicators were significantly better in the observation group than that in the control group（ P〈0. 05）. The total effective rate of the observation group was significantly higher while the incidence was significantly lower than that of the control group（ P〈0. 05）. Conclusion Chinese herbal medicine for tonifying kidney and clearing stagnation combined with aromatase inhibitor can effectively improve the symptoms and signs,reduce E2 levels,improve bone density,regulate bone turnover index,and help reduce adverse drug reactions in the treatment of postmenopausal hormone receptor-positive breast cancer.
