丹红注射液联合氯吡格雷对急性脑梗死患者血小板抑制效果及活化功能的影响

摘　　要：目的探讨丹红注射液联合硫酸氢氯吡格雷对急性脑梗死患者血小板抑制效果及血小板活化功能的影响。方法纳入80例急性脑梗死患者,随机分为两组,观察组和对照组,各40例,对照组给予常规治疗加氯吡格雷,观察组在对照组基础上加用丹红注射液治疗,共治疗2周。治疗前后比较两组NIHSS评分及Fugl-Meyer运动功能评分(FMA),应用血栓弹力图比较两组患者的血小板抑制效果。采用流式细胞仪检测血小板活化功能指标血小板糖蛋白CD63、CD62p。结果治疗后两组NIHSS评分均较治疗前明显降低(P<0.01),且观察组NIHSS评分明显低于对照组(P<0.05);治疗后两组FMA评分明显高于治疗前(P<0.01),且观察组FMA评分明显高于对照组(P<0.01)。治疗后观察组血小板抑制率[(68.53±18.32)%]明显高于对照组[(52.18±16.44)%](P<0.01)。观察组血小板抑制效果明显优于对照组(P<0.05);治疗后两组CD63、CD62p水平明显低于治疗前(P<0.01),且观察组CD63、CD62p水平明显低于对照组(P<0.01)。结论丹红注射液联合氯吡格雷能有效改善急性脑梗死患者神经功能,并且有助于抑制血小板活化功能,增强血小板抑制效果。Objective To investigate the effect of Danhong injection combined with clopidogrel on platelet inhibition and platelet activation in patients with acute cerebral infarction. Methods Eighty patients with acute cerebral infarction were enrolled and randomly divided into two groups： observation group（n=40） and control group（n=40）. The control group was given conventional therapy and clopidogrel. The observation group was treated with Danhong injection on the basis of the control group. A total of 2 weeks treatment. NIHSS and Fugl-Meyer motor function scores（FMA） were compared between before and after treatment. Thromboelastometry was used to compare the platelet inhibitory effect between the two groups. Platelet glycoproteins CD63 and CD62 p were detected by flow cytometry. Results After treatment, the NIHSS scores in both groups were significantly lower than those before treatment（P〈0.01）. The NIHSS score in the observation group was significantly lower than that in the control group（P〈0.05）. After treatment, FMA scores of the two groups were significantly higher than those before treatment（P0.01）. The FMA score in observation group was significantly higher than that in control group（P〈0.01）. After treatment, the inhibitory rate of platelet in the observation group [（68.53±18.32）%] was significantly higher than that in the control group [（52.18±16.44）%]（P〈0.01）. The inhibitory effect of platelet in the observation group was significantly better than that in the control group（P〈0.05）. After treatment, the levels of CD63 and CD62 p in the two groups were significantly lower than those before treatment（P〈0.01）. The levels of CD63 and CD62 p in the observation group were significantly lower than those in the control group（P〈0.01）. Conclusion Danhong injection combined with clopidogrel can effectively improve the neurological function in patients with acute cerebral infarction, and help to enhance the inhibitory effect of platelets.
