摘  要:目的探讨磁共振弥散加权成像应用于脑胶质瘤和脑单发转移瘤鉴别诊断中的临床价值。方法选取我院(2014年1月至2017年1月)收治的28例脑单发转移瘤患者为本研究的转移瘤组,选取同时期收治的26例脑胶质瘤患者为本研究的胶质瘤组,两组患者术前均采用磁共振成像。观察两组患者常规MRI和DWI图像特点,并计算患者瘤周水肿和肿瘤实质部分的ADC值。结果转移瘤组与胶质瘤组瘤周水肿ADC值对比无显著差异,不具有统计学意义(P>0.05);转移瘤组患者肿瘤实质部分ADC值明显低于对照组,差异具有统计学意义(P<0.05)。结论随着磁共振技术的发展,未来DWI必然会在诊断和鉴别肿瘤中运用更加广泛,可以帮助临床诊断、治疗、预后等提供更多的信息。Objective To explore the clinical value of diffusion weighted magnetic resonance imaging(DWI) in the differential diagnosis of glioma and single brain metastases. Methods A total of 28 patients with metastatic brain tumor in our hospital(January 2014 - January 2017) were selected as the metastatic tumor group. 26 patients with gliomas at the same time were selected as the glioma group. Two groups of patients underwent magnetic resonance imaging before operation. The characteristics of the routine MRI and DWI images in the two groups were observed and the patients' peritumoral edema and the ADC value of the tumor parenchyma were calculated. Results There was no significant difference in the ADC value of the peritumoral edema between the metastatic tumor group and the glioma group(P〉0.05). The ADC value of the tumor parenchyma in the metastatic tumor group was significantly lower than that in the control group(P〈0.05). Conclusion With the development of magnetic resonance technology, DWI will be more widely used in diagnosis and differential diagnosis in the future. It can provide more information for clinical diagnosis, treatment, prognosis and so on.
